摘要
目的:探讨慢性疼痛患者在全脑磁共振形态学测量(Voxel-based orphometry,VBM)以及脑静息态功能磁共振(resting state fMRI,RS-fMRI)相对于健康人脑部结构与功能方面的影像改变。方法:回顾性分析32例双侧膝骨性关节炎慢性疼痛患者(实验组)与20例健康志愿者(对照组)VBM及RS-fMRI检查结果,比较VBM、低频振幅(amplitude of spontaneous,ALFF)及功能连接指标。结果:与对照组比较,实验组患者双侧前额叶灰质明显降低,差异具有显著性(P<0.001),脑灰质及白质均未见明显增加或降低,差异无统计学意义(P>0.05);与对照组比较,实验组右侧额上回、右侧额中回、右侧扣带回及右侧岛叶均有显著差异(P<0.001);实验组全脑区均未发现ALFF值明显增加部位;与对照组比较,实验组患者双侧丘脑腹后外侧核(ventral posterolateral nucleus,VPLN)与多个脑区的功能连接存在异常,其中双侧丘脑VPLN与左侧及右侧小脑后叶、中脑和左侧海马功能连接明显增强(P<0.005);双侧丘脑VPLN与双侧前额叶和右侧枕叶功能连接明显减弱(P<0.005)。结论:VBM及RS-fMRI能够准确显示慢性疼痛患者脑区结构以及功能连接改变,临床应用价值较高。
Objective: to explore the of chronic pain patient with resting state functional mri and eoxel based morphometry. Methods :32 cases of patients with chronic pain of bilateral knee osteoarthritis and 20 cases of healthy volunteers were researched. The results of VBM and RS-fMRI were compared. Results:The bilateral frontal lobe gray matter of the experiment group were lower than that of the control group( P 〈 0.00l ) ; There were no significant difference on the ectocinerea and brain white matter( P 〈 0.001 ) ; The bilateral superior frontal gyms, right middle frontal gyrus, right mediate frontal lobe and isula of the right cerebral of the experiment group were significant difference with the control group( P 〈 0.001 ) ; There was no increase of ALFF in whole brain area of the experiment group;The two-sided cerebral ganglion VPLN and functional connectivity in multiple regions were abnormal, the two-sided cerebral ganglion VPLN and functional connectivity in multiple regions were enhanced compared with the corltrol group ( P 〈 0. 005 ) ; the two-sided cerebral ganglion VPLN and profrontal lobe and the right side of the occipital lobe were reduced compared with the control group ( P 〈 0. 005 ). Conclusion:It can show the chronic pain patient with resting state functional mri and coxel based morphometry by VBM and RS-fMRI. It is valuable in clinical application.
