摘要
摘要：目的探讨十二指肠间质瘤的临床特征及其诊断与治疗。方法回顾性分析该院2001年1月～2011年12月经手术治疗及术后病理确诊的8例十二指肠间质瘤患者的临床资料。结果临床表现无特异性，以黑便、腹部饱胀不适、嗳气、反酸、黄疸、贫血等非特异症状为主。以降部及水平部多见，2例行胰十二指肠切除术、3例行十二指肠节段切除术、2例行肿瘤局部切除、1例行远端胃大部切除术，术后回访8个月～10年，术后1例未服用伊马替尼术后6月复发，发生肝脏、腹腔转移，1年内死亡。1例间断服用伊马替尼2年，术后5年发生肝转移死亡，1例随访丢失，其余未见复发及转移。结论十二指肠间质瘤的治疗，首选手术完整切除，其术式根据肿瘤的大小、部位而采取不同的手术方式，部分患者局部切除仍可取得根治。对于高度危险患者，术后行伊马替尼辅助治疗，可取的良好效果。
[ Objective ] To analyze the features of clinical diagnosis and treatment of duodenal gastrointestinal stromal tumors. [Methods] Clinical and pathological data of 8 cases of duodenal gastrointestinal stromal tumors admitted between 2001.01.01 and 2011.12.31 in our hospital were analyzed retrospectively. [Results] The clinical manifestations were nonspecifie, with melena, abdominal discomfort, belching, acid reflux, jaundice, anemia and some other non-specific symptoms. The leaions weremainly located in the descending duodenum and the transverse portion. Pancreaticoduodenectomy was done in two cases, segmental resection was done in three cases, local resec- tion was done in two cases, distal subtotal gastrectomy was done in one case. Following resection, one case recurred of liver and peritoneal metastasis sixteen months postoperative, died within one year without using imatinib. One case died of liver metastasis five years postoperatively with using imatinib intermittent. One case failed to follow up. The rest had no recurrence and metastasis. [Conclusion] Completed surgical resection is the first choice in the treatment of duodenal gastrointestinal stromal tumors. Various surgical procedures for duodenal gastrointestinal stro- mal tumors are adopted depending on the size and location of the tumor. Some patients can be resected completely by partial resection. High-risk patients can also have good results using imatinib after operation.
