摘要
目的:探讨胺碘酮治疗阵发性心房颤动(房颤)的临床疗效。方法:将80例确诊为阵发性心房颤动患者随机分为治疗组和对照组各40例。两组患者入组后除原发病治疗药物继续维持原有用法、用量不变,停用其他抗心律失常药物。对照组给予倍他乐克。治疗组在对照组治疗基础上加用胺碘酮,第一周600mg/d,第二周400mg/d,第三周200mg/d,并从第三周开始维持剂量200mg/d治疗。两组疗程均为12周。结果:治疗组总有效率为92.50%,对照组总有效率为75.00%,两组总有效率比较,经χ2检验,差异具有统计学意义(P<0.05)。两组治疗前左心房内径分别为(36.92±1.47)mm和(36.73±1.51)mm,两组治疗后左心房内径分别为(38.27±1.49)mm和(37.45±1.43)mm,两组治疗前后组内比较均具有统计学意义(P<0.01,P<0.05);治疗后,治疗组左心房内径显著优于对照组,两组比较差异具有统计学意义(P<0.05)。结论:胺碘酮能够控制阵发性房颤的发生率,有效预防心房重构,值得临床推广。
Objective:To find out the clinical effects of amiodarone therapy on paroxysmal atrial fibrillation. Methods:Divided 80 cases of patients diagnosed with paroxysmal atrial fibrillation randomly into two groups, the treatment group with 40 cases and the control group with 40 cases. After being grouped, these patients will maintain the same usage and dosage of drugs treating primary disease and stop using other antiarrhythmic drugs. Metoprolol was given to the control group and amiodarone was given to the treatment group on the basis of control group treatment,600mg/d for the first week,400mg/d for the second week,200mg/d for the third week,and maintained the dosage of 200mg/d since the third week. The treatment course of both groups is 12 weeks. Results:The total effective rate of treatment group is 92.50%,and the total effective rate of control group is 75.00%.Contrasted the total effective rates of both groups and checked it byχ2,the difference had statistical significance (P〈0.05). Before receiving the treatment,the inner diameter of left atrial of both groups was(36.92±1.47)mm and(36.73±1.51)mm respectively;After receiving the treatment,the inner diameter of left atrial of both groups was(38.27±1.49)mm and(37.45±1.43)mm respectively. Comparison between two groups had statistical significance(P〈0.05). Conclusion:Amiodarone can control the occurrence rate of paroxysmal atrial fibrillation and prevent atrial remodeling,so it is worthful to apply it on clinical.
