摘要
目的:系统评价针刺治疗糖尿病周围神经病变(DPN)的临床疗效。方法:搜索2000年以来国内外有关针刺治疗糖尿病周围神经病变的随机对照试验(RCT),筛选出符合条件的文献并采用RevMan5.0软件进行Meta分析。结果:有16篇文献共计1?141例患者符合纳入标准,Meta分析显示,针刺治疗DPN的有效率明显好于药物组(P<0.000?01);针刺对DPN胫神经、腓总神经感觉传导速度的改善明显优于药物组(P值分别<0.000?01),对正中神经、腓总神经运动传导速度的改善显著优于药物组(P=0.03、P=0.02),对血浆NO水平的提升明显优于药物组(P=0.005)。结论:与常规西药相比针刺治疗DPN疗效突出,能明显缓解DNP症状,改善周围神经传导速度,提高血浆内NO含量。但囿于纳入文献的质量有限,有待大样本、多中心的随机对照试验提供更具说服力的证据。
Objective ： To review the curative effect of acupuncture on the diabetic peripheral neuropathy （ DPN ）. Methods ： The literatures met the conditions which were geared to randomized controlled trials（ RCT ） and published at home or abroad since 2000 were screened and analyzed by RevManS.0 software. Results ： 16 papers, a total of 1141 patients met the inclusion criteria. The Meta-analysis showed that the efficiency of＇ acupuncture treatment on DPN was better than that of the drug group （ P〈0.00001 ）. The improvement of tibial and peroueal nerve＇s sensory conduction velocity from acupuncture on DPN was significantly better than that of the drug group （ P〈0.00001, respectively ）. The motor conduction velocities of median and peroneal nerve were obviously faster than that of the drug group （ P=0.03, P=o.02 ） and the plasma NO level was notably higher than that of the drug group （ P=0.005 ）. Conclusion ： Compared with the conventional western medicine, the curative effect of acupuncture on DPN is outstanding. Acupuncture treatment can obviously relieve DNP symptoms, improve the conduction velocities of peripheral nerves and increase the content of NO in plasma. However, due to the limited quality of documents, more randomized controlled trials of large sample and multi center were needed to provide more convincing evidence.
