颈动脉超声Crouse积分法评价颈动脉斑块与血压、血脂及血尿酸的关系 被引量:30

摘要 目的 应用Crouse斑块积分法量化评价颈动脉硬化(CAS)的严重程度,并以Crouse斑块积分值分组进行血压、血脂及血尿酸(BUA)水平的比较和相关性分析,探讨血压、血脂及BUA水平与CAS发生及严重程度的关系.方法 研究对象为我院进行健康体检的261名城镇居民,年龄35~75岁.应用颈动脉彩色多普勒超声测量并记录双侧颈动脉内各个独立斑块的厚度并将所测得的所有独立斑块厚度相加后作为Crouse积分.同时检测记录血压值和血清甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)及BUA水平.依据颈动脉超声Crouse积分结果将261例受试者分为正常对照组140例(n=140),内中膜厚度(IMT)增厚组44例(n=44)和斑块形成组77例(n=77).并分别对3组的血压、TC、TG、HDL-C、LDL-C、BUA进行组间两两比较及多元逐步回归分析.结果 IMT增厚组、斑块形成组的收缩压(SBP)、舒张压(DBP)、TC、LDL-C及BUA水平均高于正常对照组(均P〈0.05),而TG、HDL-C水平与正常对照组比较差异无统计学意义(P〉0.05);斑块形成组的SBP、LDL-C及BUA水平显著高于IMT增厚组(均P〈0.05),而两组间TG、TC和HDL-C水平差异无统计学意义(均P〉0.05).在斑块形成组中,受试者的SBP、LDL-C和BUA水平与Crouse积分水平呈显著的正相关关系(均P〈0.05),而性别、吸烟史、糖尿病史、DBP、TG、TC、HDL-C水平与Crouse积分均没有相关关系(均P〉0.05).进一步通过比较标准化回归系数值发现:上述三者对Crouse积分的影响程度由大到小依次为LDL-C(b=0.397,P〈0.01)、SBP(b=0.376,P〈0.01)和BUA(b=0.220,P〈0.05).结论 应用颈动脉超声Crouse积分法在颈动脉斑块定量评价上可靠性相对较高;SBP、TC、LDL-C及BUA水平均与CAS的发生有较密切关系,而HDL-C、TG和 DBP水平与CAS关系不确切.BUA是影响CAS严重程度的� Objective The Crouse plaque score was used to quantitatively evaluate the seventy ot the carotla artery atherosclerosis(CAS), and the subjects were divided into identified groups according to the Crouse plaque score. This aim was to explore the relationship of CAS with the blood pressure,lipidemia and blood uric acid(BUA) evaluated hy Crouse score with carotid ultrasonography. Methods The study involved 261 urban residents from healthy body check in the hospital, aging from 35 to 75 years old. Using color Doppler ultrasound, we measured and recorded the thickness of the bilateral separate plaques in the carotid artery in turn, and added all measured independent plaque thickness together, then served as Crouse score. Meanwhile, we detected and recorded the blood pressure value, the level of triglyceride ( TG), total cholesterol ( TC), high density lipoprotein cholesterol ( HDL-C), low density lipoprotein- cholesterol(LDL-C) and BUA. In accordance with the Crouse score of carotid artery ultrasound, 261 subjects were randomly assigned to normal control group ( n = 140), intima-media thickness(IMT) thickening group( n = 44), and plaque formation group( n = 77). Furthermore, we made comparisons among normal control group, IMT thickening group and plaque formation group on inter-group level about blood pressure, TC,TG, HDL-C, LDL-C and BUA, as well as multiple stepwise regression analysis,respectively. Results SBP,DBP,TC, LDL-C and BUA level of IMT thickening group and plaque formation group were significantly higher than those of normal control group (all P〈 0.05), while TG and HDL-C level of the former two groups showed no significance compared with those of normal control group( P〈 0.05);the SBP, LDITC and BUA level of plaque formation group were significantly higher than those of IMT thickening group (all P〈 0.05), while there were no statistical significance in the level of TG, TC and HDL-C between plaque formation group and IMT thickening group (all P�
