逆行腋窝淋巴结示踪术对预防乳腺癌术后上肢淋巴水肿的作用 被引量:26

摘要 目的探讨以纳米炭混悬液示踪上肢来源的腋窝淋巴组织的可行性,以及在乳腺癌腋窝淋巴结清扫术中保全该示踪淋巴组织后对预防上肢淋巴水肿的有效性。方法 70例分期为TxN0M0的乳腺癌患者分成两组,每组35例,ARM组术前以纳米炭混悬液行经上臂逆行腋窝淋巴结示踪术(ARM),成功示踪的有34例,34例患者行保留示踪淋巴结(管)的腋窝淋巴结清扫术加乳房切除术,未显影的1例与传统组35例行经典腋窝淋巴结清扫术加乳房切除术,术后1年内分别观察两同侧上肢淋巴水肿的发生情况。结果 ARM组成功率高达97.1%,ARM组和传统组行腋窝淋巴结清扫术后1年内发生淋巴水肿的患者分别为1例、14例,发生率差异有统计学意义。结论纳米炭混悬液ARM是可行的,术中保全上肢来源的示踪的淋巴组织能减少损伤上臂回流的淋巴管道,有效降低术后同侧上肢淋巴水肿的发生率。
