摘要
目的研究肌萎缩侧索硬化症(amyotrophic lateral sclerosis,ALS)患者三重经颅磁刺激(triple stimulation technique,TST)改变,比较TST与传统电生理指标的诊断价值。方法收集我院门诊和病房收治的ALS患者50人进行TST测定及传统电生理指标测定。结果 ALS组50例患者TST波幅比阳性率为62.0%,与健康对照组(阳性率为9.1%)比较有显著性差异(χ2=17.227,P=0.000)。有UMN体征ALS组(28例)、无UMN体征ALS组(22例)和正常对照组(20例)TST波幅比异常率分别为89.3%、27.3%,9.1%,TST波幅比检测ALS患者UMN损害的敏感度更高,为89.3%、准确度90.0%均高于传统电生理指标,特异度90.9%,低于传统电生理指标。结论 TST波幅比可以成为判定ALS患者临床UMN损害的敏感指标,优于传统电生理指标。
Objective:To estimate upper motor neuron (UMN) lesion with the triple stimulation technique (TST) and other TMS parameters in amyotrophic lateral sclerosis (ALS). Methods: Fifty ALS patients were enrolled in the study. Patients were examined clinically, with conventional transcranial magnetic stimulation (TMS), and TST. Results: The abnormal rates of the TST amplitude in ALS with UMN signs, ALS without UMN signs and controls were 89.3%、27.3%,9.1% respectively. The sensitivity of TST amplitude ratio in detecting UMN lesions was higher than other TMS parameters. Conclusion: TST appears to be an accurate and sensitive measure of detecting and quantifying UMN conduction failure in ALS than other parameters.
