摘要
目的声门下吸引是预防呼吸机相关性肺炎（ventilator associated pneumonia，VAP）的重要措施。为了解ICU护士声门下吸引执行率的现状，探讨提高声门下吸引执行率的措施。方法采用现场观察法，将9名经过声门下吸引培训的护士作为观察者，随机分配到普通外科监护室、重症医学科监护室、急救医学科，每科3名，观察我院69名ICU护士在白班（08：00、12：00、16：00）和夜班（20：00、24：00、04：00）各时间点声门下吸引执行率情况。结果1CU护士总体平均声门下吸引执行率为76．10％，其中自班声门下吸引执行率高于夜班（81．79％us70．00％，P〈0．01）；气道专职护士执行率高于非气道专职护士执行率（89．04％w70．85％，P〈0．01）；护士年资1-2年、6-9年、≥10年，执行率分别为62．88％、74．96％、81．06％、91．40％，两两比较差异有统计学意义（P〈0．01），且随年资增加执行率呈上升趋势。结论ICU护士声门下吸引执行率有待提高，应合理分级配置人力资源，加强人工气道规范化培训，持续改进人工气道护理质量。
Objective Subglottic secretion drainage is an important measure to prevent ventilator-associated pneumonia. This study was to gain a clear awareness of the execution rate of SSD among ICU nurses and discuss the measures for its improvement. Methods Site investigations were conducted on the execution of SSD by 69 ICU nurses at 6 different time points. Results The to-tal rate of SSD execution by the ICU nurses was 76.10%, significantly higher on the day shift （81.79%） than on the night shift （70.00%） （P 〈0.01 ）, and so was it among the nurses specially trained in respiratory tract care （89.04%） than among the ordinary nurses （70, 85 % ） （ P 〈 0.01 ）. Statistically significant differences were found in the SSD execution rate among the nurses with differ-ent lengths of service （P 〈 0.01 ）, which showed a rising trend with the increasing length of service. Conclusion The total rate of SSD execution by ICU nurses has yet to be further improved, and the purpose can be achieved by rational allocation of nurses with dif-ferent service records and enhanced training in respiratory tract care.
