摘要 目的:探讨高迁移率组蛋白1(high mobility group box1,HMGB1)在狼疮性肾炎(lupus nephritis,LN)患者肾组织中的表达及其临床意义。方法:LN患者63例,B超引导下行肾穿刺活检术获得肾脏组织标本,外伤性肾粉碎性破裂患者切除的正常肾组织15例,用免疫组织化学SP法检测HMGB1蛋白在肾脏组织中的表达,并分析其与肾脏活动指数(AI)、慢性指数(CI)和LN病理分型之间的关系。结果:LN组的HMGB1蛋白表达显著高于对照组(t=9.263,P<0.05);其表达水平与系统性红斑狼疮活动性指数(SLEDAI)评分(r=0.579,P<0.05)、AI(r=0.708,P<0.05)和肾小管间质病变(TIL)评分(r=0.815,P<0.05)呈正相关,与CI评分呈负相关(r=0.582,P<0.05)。结论:HMGB1可能参与了LN发病过程,促进了LN的病情进展。 Objective: To evaluate the clinical significance of high mobility group box (HMGB) 1 expression on the renal tissues of patients with lupus nephritis(LN). Methods: The renal tissues of sixty-three patients with LN and renal tissues of fifteen patients with splintered fracture were studied by immunohistochemistry for HMGB1 protein expression. The correlation between the HMGB1 protein expression and the renal active index(AI), chronicity index(CI), pathologic typing of LN were analyzed. Results:The HMGB1 expression level of the LN groups was higher than that of the control group(t=9.263, P〈0.05). It showed positive correlation between HMGB1 expression and SLE DAI score(r=0.579, P〈0.05), AI(r=0.708, P〈0.05) and pathologic typing score(r=0.815, P〈0.05), and negative correlation between HMGB1 expression and CI score(r=0.582, P〈 0.05). Conclusion: HMGB1 may be involved in the process of pathogenesis of LN and contributed to the progression of patient's condition.
