摘要
目的探讨门静脉高压症断流术后门静脉系统血栓形成的各种因素。方法回顾性分析我院普外科近3年诊断为肝炎后门静脉高压症接受脾切除+断流术治疗的86例患者的临床资料,观察其术后PVT形成情况,并对术后PVT形成的因素进行分析。结果全组术后PVT发生率为32.6%;PVT形成与患者年龄、肝功能分级,食管胃底静脉曲张程度、术前门静脉直径,术后门静脉血流速度,术后血小板值等因素相关(均P<0.05),而与患者性别、术前门静脉血流速度,术后PT值无关(均P>0.05)。结论年龄、肝功能分级,食管胃底静脉曲张程度、术前门静脉直径,术后门静脉血流速度,术后血小板值是脾切除+断流术后PVT形成的影响因素。
Objective To investigate the incidence and causes of postoperative portal vein thrombosis （PVT） in patients after devascularization operations.Methods The clinical data of 86 patients with cirrhotic portal hypertension undergoing splenectomy and gastric pericardial devascularization over 3 years were reviewed.The incidence of PVT after operation was observed and the influential factors for PVT were analyzed.Results and Conclusions The incidence of PVT after operation in the entire group was 32.6％.Statistical analysis showed that the postoperative PVT was associated with patients＇ age,degree of esophagogastric varices,hepatic function grade,portal pressure,postoperative platelet （all P ＜ 0.05）,but it was irrelevant to the sex,values and portal vein blood flow velocity as well as the the postoperative PT values of the patients （all P ＞ 0.05）.
