摘要
目的分析自体骨打压植骨在股骨颈骨折的初次生物型全髋关节置换术中的临床疗效。方法选取我科2008年6月-2012年12月期间治疗的股骨颈骨折患者全髋关节置换手术64例，根据治疗方法分为2组，观察组（自体骨打压植骨的初次生物型全髋关节置换术）32例，对照组（未使用打压植骨的初次全髋关节置换术）32例，所有病例都经过6～36个月的随访，平均随访18．7个月，统计术后髋关节早期及末次随访时Hariis评分及VAS评分，复查X片观察髋臼下沉及松动情况并采用SPSS15．0进行统计分析。结果2组在早期的Harris评分及Ⅵ峪评分无明显差别，而随访末期的评分差异有统计学意义。结论自体骨打压植骨在股骨颈骨折的初次生物型全髋关节置换中有明确的效果，远期随访中的假体的稳定性明显增大，下沉溶解率明显降低，临床疗效满意，其高保留骨量将逐渐成为今后关节置换的趋势。
Objective To explore the clinical efficacy of impaction autogenous bone graft in the treatment of transcervical fracture patients with primary bio- total hip arthroplasty. Methods 64 transcervical fracture patients treated with total hip arthroplasty were collected and divided in to observation group （n = 32, impaction autogenous bone graft in primary total hip arthroplasty）and control group （ n = 32, simply primary total hip arthroplasty）. All patients were conducted with fol- low - up ranged from 6 months to 36 months. Hip Harris score and VAS score during the early and last follow- up were calculated, and the acetabular loosening and sinking in the X - ray were ob- served. Results There were no significant difference of the early Harris score and VAS score be- tween two groups, but there were significant differences of Harris score and VAS score in final stage of follow- up. Conclusion Effect of impaction autogenous bone graft on treatment of the transcer- vical fracture patients with primary total hip arthroplasty is remarkable. Stability of prosthesis in- creases apparently in long - term follow- up, while rate of sinking dissolution reduces obviously. Based on its feature of high bone mass preservation, it will be gradually widely used instead of joint replacement in clinic.
