摘要
目的:观察应用自酸蚀粘接系统充填老年重度磨损磨牙的临床观察。方法:选择口内同时具有两颗第一磨牙牙合面重度磨损的患者52例,共104颗患牙做为研究对象。采用自身对照法,将同一患者口内随机选择一颗第一磨牙为实验组(共52颗),采用自酸蚀粘接系统+3MP60树脂直接充填修复;另一颗第一磨牙为对照组(共52颗),采用磷酸酸蚀系统+3MP 60树脂直接充填修复。评价标准采用改良USPHS/Ryge评价标准,采用χ2检验,于充填即刻、3、6、12及24个月复查评价。结果:充填即刻、术后3个月、6个月、12个月、24个月复查实验组与对照组在修复体表面情况、边缘密合性、边缘着色、色泽协调性、继发龋方面均无统计学差异(P>0.05),充填术后3个月复查牙髓反应实验组与对照组有统计学差异(P<0.05),术后6个月、12个月、24个月复查均无统计学差异(P>0.05)。结论:自酸蚀粘接系统与全酸蚀粘接系统在修复老年人磨牙牙合面重度磨损病例中具有同等的粘接效果,自酸蚀粘接系统可降低术后敏感。
Objective: To evaluate the clinical effect of restoration with self-etching adhesive bonding system for the molars with abrasion in elderly patients. Methods: 104 the first molars from 52 patiens with two molars in each patients were involved in the study. Self-control method was used. One molars in each patient was randomly assigned into the experimental group (52 teeth), which was restored with Clearifil self-etching bond (SE-Bond) and a composite resin(3M P60). Other molars were included in the control group (52 teeth ), which was treated with phosphoric acid etching adhesive system (Prime & Bond NT) and a composite resin (3M P60). After restoration immediately, 3 months, 6months, lyear, 2years, the clinical effects were evaluated according to the modified USPHS/Ryge criteria, and X^2 statistics was performed. Results: At different time points, there was no significant difference between each two groups at surface morphology of restoration, marginal adaptation, marginal discoloration and secondary caries (P 〉0.05). But post-operative sensitivity between two groups were found to have significant differences in 3 months after treatment(P〈 0.05); while there was no significant difference in 6, 12, 24 months after treatment. Conclusion: This study showed that both adhesive systems was excellent on restoration molars with abrasion in elderly patients during two years clinical trial. However, self-etching bond adhesive system caused less post-operative sensitivity than phosphoric acid etching adhesive system during 3 months.
