缺血性脑卒中患者急性期血浆胎盘生长因子水平及其与NEW-TOAST病因学分型相关性的研究 被引量:2

Studies of Plasma Placental Growth Factor Level in Patients with Acute Ischemic Stroke and Its Relationship with NEW-TOAST Subtypes

摘要 目的探索缺血性脑卒中患者急性期血浆胎盘生长因子(pPlGF)水平的变化及其临床意义。方法以急性缺血性脑卒中患者60例(AIS组)和其他神经系统疾病患者20例(对照组)为研究对象,采用酶联免疫吸附测定法(ELISA)检测pPlGF水平,分析各组pPlGF水平的差异,以及pPlGF与患者年龄、NEW-TOAST病因学分型及预后等临床因素的相关性。结果 AIS组pPlGF水平较对照组显著升高(277.63±277.12 vs 72.88±82.57)pg·mL-1(P<0.001);校正年龄及性别后,AIS组pPlGF水平仍显著高于对照组(P=0.041)。NEW-TOAST各亚型中,动脉粥样硬化血栓型[AT:(246.58±234.50)pg·mL-1,P=0.003]及心源性脑栓塞型[CE:(487.67±510.38)pg·mL-1,P=0.03]患者pPlGF显著升高;校正年龄及性别因素后,AT及CE型pPlGF水平仍显著高于其他亚型(P=0.026)。pPlGF水平与年龄(P<0.001)及起病后14 d的NIHSS评分(P=0.015)呈显著相关,但纳入相关因素校正后呈无相关性。结论 pPlGF可能参与缺血性脑卒中急性期的病理生理过程;pPlGF在不同病因的AIS发病过程中的权重不同;NEW-TOAST病因学分型AT亚型患者的pPlGF变化以及对预后的影响可能有一定临床价值。 Aim To investigate the changes of plasma placental growth factor (pPlGF) in patients with acute ischemic stroke (AIS) and its clinical treatment value.Methods pP1GF levels were examined by enzyme-linked immunosorbent assay (ELISA) in 60 AIS patients and 20 controls with other neurological diseases.The variances of pP1GF in the two group and the relationships between pPlGF and age,TOAST subtypes and prognosis were analyzed.Results Levels ofpPlGF increased significantly in AIS patients than that in the controls (277.63 ± 277.12 vs 72.88 ± 82.57 pg·mL-1,P〈0.001),and the difference was still significant after age and sex (P=0.041) being controlled.The level of pP1GF increased prominently in atherothrombosis (AT,246.58 ± 234.50 pg·mL-1,P=0.003) and cerebral embolism (CE,487.67 ± 510.38 pg·mL-1,P=0.03) subtypes of NEW-TOAST classification and the difference was still significant after age and sex being controlled (P=0.026).The level of pP1GF was positively correlated with age (P〈0.001) and NIHSS at day 14 after onset (P=0.015).Whereas there was no significance after other factors being controlled (P〉0.05).Conclusion pP1GF was demonstrated to play a role in the mechanism of AIS,especially in AT and CE subtypes in the aspect of clinic,and being different weights in different mechanism of AIS.Further investigation on pP1GF in AT patients and its effect on AIS prognosis may have clinical treatment value.
