摘要
目的探讨手术治疗急性跟腱断裂的疗效和并发症。方法对2009年1月～2012年12月收治的112例急性跟腱断裂患者，采用单纯端端缝合修补89例，腓肠肌筋膜瓣加强修补23例。男性98例，女性14例；年龄23～48岁，平均37岁。闭合性损伤101例，开放性损伤11例。结果本组112例，平均随访时间为14．5个月（6～18个月）。根据美国足踝骨科协会（AOFAS）评分标准，平均分为92．2分（74～100分）。其中优63例，良39例，可10例。踝关节活动度患足为（55．2±7．1）°，健足（57．1±5．8）°，两者无显著差异（t=0．101）。采用单纯端端缝合修补患者的AOFAS评分为90．9±10．3，加强修补患者的AOFAS评分为93．6±9．9，两者无显著差异（t=0．389）。踝关节活动范围分别为（54．2±9．3）°和（56．1±7．8）°，两者也无显著差异（t=0．764）。切口表浅感染5例，深部感染1例，跟腱再断裂2例。结论手术治疗急性跟腱断裂疗效可靠、并发症少。
Objective To investigate the efficiency and complications of the operative repair in treating acute Achilles tendon ruptures. Method A total of 112 acute Achilles tendon ruptures patients were admitted into our hos- pital from Jan. 2009 to Dec. 2012, including 98 males and 14 females in the 23 - 48 years range （ mean 3 years old）. Of 112 cases of acute Achilles tendon ruptures ,89 cases were repaired with simple end-to-end suture and 23 cases were repaired and then augmented with gastrocnemius-soleus fascia. The ankle function was evaluated according to the AO- FAS criteria. The ankle movement range including plantar flexion and dorsal flexion was measured. Complications including wound infection,re-rupture,sural nerve damage and heel ulcer were studied. Results The follow-up ranged from 6 to 18 months, averaging 14. 5 months. The mean score of AOFAS was 92.2（74-100 ）, and 63 eases were considered excellent,39 good,and 10 fair. The ankle joint activity on the injured side was（55.2 ±7.1 ）°and （57.1 ±5.8）° on the normal side,which had no significant difference. The ankle movement range on the injured side was（ 54. 2 ± 9. 3 ）° and （ 56. 1 ± 7. 8 ）° on the normal side, indicating no significant difference. Superficial wound infection, deep wound infection and re-rupture were detected in 5 cases, 1 case and 2 cases, respectively. Conclusion Operation for treating acute Achilles tendon rupture can obtain satisfactory clinical outcomes and less complications.
