摘要
目的探讨青少年腰椎间盘突出症的临床特征及经皮激光气化减压术(percutaneous laser disk decompressuer,PLDD)治疗的远期临床疗效。方法青少年腰椎间盘突出症患者36例。采用PLDD治疗,术后平均随访12～36个月,比较术前,术后12、24和36个月手术前后疼痛的VAS评分,改良Mcnab疗效评分法评估疗效。结果术后VAS疼痛评分较术前明显缓解,差异具有显著意义(P<0.05),术后24和36个月随访满意率(Mcnab评分优良者)分别为91.2%和90.6%。结论青少年腰椎间盘突出症具有典型的根性腿痛、软性的椎间盘突出、症状持续时间短的特征,掌握手术适应证激光汽化减压术安全且有效改善临床症状,术后积极功能锻炼,远期效果好。
Objective To explore clinical featuresof adolescent lumbar disc herniation and the long-term clinical efficacy of percutaneous laser discdecompression（PLDD）. Methods The follow-ups for different periods from 12 to 36 months were conducted for 36 young patients having received PLDD treatment. The VAS scores in the 12th,24th and 36th month were compared and the efficacy was evaluated with improved Mcnab evaluation method. Results The VAS pain scores were improved greatly after the operation,and the difference between preoperative score and postopera-tive score was statistically significant（P 0. 05）. The 24 months＇ and 36 months＇ follow-up showed a satisfaction rate（having an excellent Mcnabrate） of 91. 2% and 90. 6% respectively. Conclusions Adolescentlumbar disc herniation features severe radicular leg pain,soft disc herniation and shorter duration of symptoms. For cases applicable,laser vaporization decompression is safe and effective in improving the symptoms and the long-term effect is good based on active postoperative functional exercise.
