原发性乳房外Paget病中HER-2的表达

摘要 目的:观察原发性乳房外Paget病中HER-2/neu基因扩增及HER-2蛋白的表达,探讨其在乳房外Paget病中的意义。方法:收集2007年1月—2011年9月原发性乳房外Paget病手术切除标本20份,应用显色原位杂交和免疫组织化学的方法,分别检测患者皮损组织石蜡包埋标本中HER-2/neu基因扩增和HER-2蛋白表达情况。结果:所有20例患者分为3组:免疫组织化学检测HER-2蛋白高表达组、低表达组和阴性表达组分别为8例、6例和6例,HER-2蛋白阳性表达率为70.0%。显色原位杂交检测高扩增组、低扩增组和无扩增组分别为7例、2例和11例,HER-2基因的扩增率为45.0%。采用Spearman秩相关进行分析,两种检测方法的结果具有正相关性(P<0.05)。结论:在原发性乳房外Paget病中存在HER-2/neu基因扩增和HER-2蛋白的表达,两者之间存在正相关性。干扰HER-2蛋白的表达可能为临床治疗原发性乳房外Paget病提供一种新的策略。 Objective: To observe the HER-2 protein expression and HER-2/neu gene amplification in primary extramam-mary Paget’s disease(EMPD) and to investigate HER-2’s significance in Paget’s disease. Methods: A total of 20 cases of EMPD were enrolled into the study from Jan. 2007 to Sep. 2011. Immunohistochemistry(IHC) and chromogenic in situ hy-bridization(CISH) methods were used to detect the HER-2 protein expression and HER-2/neu gene amplification in paraffin-embedded surgical specimens respectively. Results:The results of HER-2 protein were divided into three groups:high expres-sion(8/20), low expression(6/20) and negative expression(6/20) by IHC. The positive expression is up to 70%. The results of HER-2/neu gene were also divided into three groups: high amplification(7/20), low amplification(2/20) and negative amplifi-cation(11/20) by CISH. The positive amplification reaches 45%. The correlation of HER-2 protein measured by IHC and HER-2/neu gene detected by CISH with Spearman rank correlation analysis , the result showed that the correlation of two methods is positive(P&lt;0.05). Conclusions: HER-2 protein expression and HER-2/neu gene amplification are existed in most cases of EMPD. The interference of HER-2 protein expression may provide a new strategy for the clinical management of EMPD.
