摘要
目的探讨血管生成拟态(vasculogenic mimicry,VM)表达对原发性肝癌(primary liver cancer,PLC)患者预后的影响。方法收集161例临床病理资料完整的PLC术后标本,15例癌旁组织做为对照。采用CD31-PAS双重免疫组化染色观察VM的表达,并进行统计分析。结果161例PLC患者VM表达率为37.9%(61/161),对照组15例无VM表达。VM表达阳性者中位生存时间较阴性者少18.6月,1、2、3年的生存率分别为56%、40%和26%,生存分析显示其与VM表达阴性者差异有统计学意义(Logrank=10.705,P:0.001)。关联性分析结果显示VM的表达与PLC的TNM分期、肿瘤大小及肝细胞癌Edmondson分级有关(P值分别为0.025、0.032和0.006);肝硬化与VM的表达存在明显的相关性(χ2=8.284,P=0.004);Ⅲ~Ⅳ期、同一肿瘤体积分组(≥5cm,〈5cm)PLC患者以及HCC患者1、2、3年的生存率与VM的表达存在明显的相关性。结论PLC中存在VM表达,VM表达阳性者生存时间短于VM阴性者,VM可能是影响原发性肝癌患者预后生存率的一个因素。
Objective To explore whether vasculogenic mimicry (VM)has an influence on prognosis of patients with primary liver cancer(PLC). Methods Complete clinical and pathological data of 161 PLC cases were collected in this study. Expression of VM were determined by using an immunohistochemical double staining of CD31 and PAS. All surgical patients were followed-up. Results The positive rates of VM in the primary liver cancer group and the control groups were 37. 9% (61/161) and 0 % (0/15) ,respectively. The 18.6-month survival rate of the VM positive group was lower than that of the VM negative group. The results of life-table showed that 1-,2-and 3-year survival rates of the VM positive group were 56% ,40% and 26% ,respectively. Survival analysis showed that the difference was statistically significant between the VM positive and negative groups ( Log-Rank = 10. 705, P = 0. 001 ). The law analysis indicated that the expression of VM was positively correlated with TNM stage, tumor size, HCC Edmondson stage (P = 0. 025,0. 032 and 0. 006,respectively). The expression of VM was significantly upregulated in the PLC with cirrhosis (X2= 8. 284,P = 0. 004). Moreo- ver,there was significant difference in the 1-,2-and 3 years survival rate and the VM expression between patients with hepatocellular carcinoma(HCC) and PLC at III-IV stage, with the same tumor size group (5 cm or more, 〈 5 cm)or the same pathological types. Conclusion There is expression of VM in patients with PLC. The survival time of patients with VM positive is shorter than that of patients without VM expression. Maybe VM is one of factors influencing prognosis and survival rates of patients with PLC.
