摘要
目的应用血栓弹力图(TEG),探讨毒蛇咬伤患者体内凝血状态。方法 86例毒蛇咬伤患者,50例正常对照,均作血常规血小板计数和TEG检测。结果绝大多数(74/86)毒蛇咬伤患者TEG表现为低凝图象:R值7.7±1.9,K值6.8±4.7较正常对照组明显延长,α角38.8±16.3较正常对照组明显减小,Ma值38.2±16.5较正常对照组明显变窄(P<0.01),CI值-7.4±5.2较正常对照明显减低。结论毒蛇咬伤患者体内存在病理性低凝状态,临床可根据TEG的变化诊断毒蛇咬伤患者的凝血状态,对判断病情,预后及指导临床治疗具有重要价值。
Objective Thrombelastometry （TEG）explore the snakebite patients coagulation sta- tus. Methods 86 cases of snakebite patients,50 cases as normal controls, were as blood platelet count and TEG testing. Results Vast majority（ 74/86 ）TEG snakebite patients showed low freezing images：R value of 7.7 ± 1.9, K value of 6. 8 ± 4.7 compared with normal control group was significantly prolonged, α angle of 38.8 ± 16. 3 compared with normal control group was significantly less small, Ma value 38.2 ± 16.5 compared with normal control group was significantly narrower （ P 〈 0. 01 ） , CI values- 7.4 ± 5.2 was significantly re- duced compared with normal control. Conclusion snakebite patients is the existence of pathological low coag- ulable state,according to the clinical diagnosis of snakebite TEG changes in the patient＇s coagulation status, right to determine the condition, prognosis and guide clinical treatment of great value.
