摘要 目的 探讨高眼压状态下白内障晶状体进行超声乳化手术的可行性及临床效果分析.方法 对19例19眼持续高眼压状态下的急性闭角型青光眼合并白内障行晶状体超声乳化术,随访6个月~2年,观察术后视力、眼压、前房的变化及术中术后并发症情况.结果 19只眼术后视力均有所提高,矫正视力:光感~0.04者2只眼,0.05 ~0.25者4眼,0.3 ~0.5者11眼,0.6~1.0者2眼.术后1周内眼压仍有波动,应用药物可控制眼压.术后1个月眼压全部在正常范围,其中2只眼仍需滴用降眼压药物.术后14只眼前房明显加深,4只眼前房稍浅,其中1只眼在术后1周内前房深度有波动.结论 在高眼压状态下行晶状体超声乳化手术是有效的. Objective To investigate the clinical efficacy of phacoemulsification for cataract in stat of high intraocular pressure.Methods The phacoemulsification with posterior chamber intraocular lens implantation were performed in 19 cases (19 eyes) of cataract with glaucoma under high intraocular pressure.The post-operative follow up was 3 months to 2 years,and the vision,intraocular pressure,change of anterior chamber depth and complications were observed.Results The visual acuities of all cases were improved.The best corrected visual acurities were < 0.05 in 2 eyes,between 0.05 and 0.25 in 4 eyes,between 0.3 and 0.5 in 11 eyes,and between 0.6 and 1.0 in 2 eyes.The post-operative intraocular pressure recovered to normal in all eyes after one month with 2 cases treated by antiglaucoma medications.The postoperative anterior chamber depth was increased in 14 eyes,anterior chamber depth is still shallow in 4 eyes,anterior chamber depth instable in 1 eye.Conclusion The phacoemulsification with posterior chamber intraocular lens implantation in the management of acute angle closure glaucoma under high intraocular pressure is safe and effective.
