摘要
目的分析本院静脉用药调配中心(pharmacy Intravenous Admixture Services,PIVAS)发生的差错事故,制定防范措施,保障患者安全、及时用药。方法回顾性调查濮阳市油田总医院静脉用药调配中心在2012年1月1日-2012年4月1发生的差错,找出差错原因,以杜绝此类差错事故的再次发生。结果 PIVAS自2012年1月1日-2012年4月1从289650袋(例)的总工作量中检出237例差错。主要表现在打印处方问题(12%)、审核医嘱差错(8%)、排药差错(41%)、核对差错(14%)、配置差错(5%)、打包及药品装箱差错(8%)、临床护士验收药品后的相关问题(12%)等七个方面。结论对PIVAS的管理进行细化、精化,找出各类问题根源及应对和解决措施,对实现安全、准确、高效的静脉药物配置,具有重大意义。
Objective To develop preventive measures by analyzing the mistake accidents in Pharmacy Intravenous Admixture Services （PIVAS） in the hospital to ensure the patients receive timely and safety treatment. Methods The mistake accidents from Jan.l 2012 to Apr.1 2012 were collected and analyzed in our PIVAS center and the reasons were found out to avoid the homologous mistakes. Results 237 mistake accidents were found out in 289650 infusion bags. The mistakes can be classified by the following seven reasons such as printing （12%）,prescription auditing （8%）. medicine rowing （41%）, checking （l 4%）, infusing （5%）, sending errors （8%） and other problems from the wards （12%）. Conclusion It is important to realize high-performance, safe and accurate infusing by finding out the reasons, resolving methods and detailed management for PIVAS.
