摘要
慢性肾衰属于《金匮要略》虚劳病,慢性肾衰的基本病机是肾气大亏,脾肾虚衰,浊瘀内停,多表现气血阴阳俱虚。其治疗规律:首先补脾胃、建中气,可在小建中汤、黄芪建中汤基础上加党参、白术、茯苓等;二是重视补益脾肾二脏,以肾气丸为代表方,可加枸杞子、菟丝子、冬虫夏草等;三是扶正祛邪,虚实并治,以薯蓣丸为代表方,可加大黄、芒硝等,以加强泄浊排毒之功;四是侧重甘温扶阳,重用附子、桂枝、黄芪、生姜等药,此治法在临床上对于慢性肾衰病人而阳虚偏甚者最为适宜。进一步探讨和研究《金匮要略》虚劳理论,对现代慢性肾功能衰竭的防治具有重要的临床指导意义。
Chronic renal failure is consumptive disease of Synopsis of Golden Chamber. The basic pathogenesis of chronic renal failure is kidney deficiency, deficiency in spleen and kidney, phlegm stasis within the body, mainly reflected in yin, yang, qi and blood deficiency. The treatment rules of chronic renal failure： First of all, nourishing spleen, stomach and middle qi, adding Codonopsis pilosula, the rhizome of large-headed atractylodes, tuckahoe based on Xiao Jianzhong Decoction and Huangqi Jianzhong Decoction; Secondly, focusing on nourishing spleen and kidney, Shenqi Pill as the representative prescription, plus barbary wolfberry fruit, dodder, cordyceps sinensis; Thirdly, strengthening body resistance to eliminate evil, treating both deficiency and excess, Shuyu Pill as the representative prescription, plus rhubarb, mirabilite etc, to reinforce the function of discharging turbid and detoxifying; Fourthly, emphasizing on sweet and warm drug nourishing yang, using medicine as radix aconiti praeparate, cassia twig, astragalns membranaceus, common ginger and other drugs, which is more suitable for chronic renal failure patients with kidney yang deficiency in clinic. Further discussing and researching deficiency theory in Synopsis of Golden Chamber has important clinical guiding meaning, which will provide references for preventing and treating chronic renal failure.
