摘要 目的探讨甲状腺微小癌(Thyriod microcacinoma,TMC)的超声结构特性,以提高其诊断准确率。方法回顾性分析30例经手术和病理证实的TMC的超声结构特性。结果 TMC 30例共34个癌结节。位于右叶19个,左叶12个,峡部3个;病灶直径2~10 mm;32个癌结节(94%)表现为边缘不规则可见毛刺;26个(76%)内可见"针尖样"微钙化;6例(20%)颈部淋巴结转移,其中2例淋巴结内出现"针尖样"微钙化。其中微小乳头状癌29例(33个结节),甲状腺微小髓样癌1例(1个结节)。结论 TMC的边缘毛刺、"针尖样"钙化具有一定的特征性,对诊断TMC具有重要价值。 Objective To study the ultrasonic structural characteristics of thyroid microcacinoma (TMC), in order to improve the diagnostic accuracy of TMC. Methods The ultrasonic structural characteristics of TMC were ret- rospectively analyzed in 30 patients (34 cancer nodules) confirmed by surgery and pathological changes. Results In the 30 patients, a total of 34 cancer nodules were found, with small papillary microcarcinoma 29 cases (33 nodes) and medullary thyroid microcarcinoma 1 case (1 nodes), of which 19 located in the right lobe, 12 in the left lobe and 3 in the isthmus. The lesions were 2-10 mm in diameter. Thirty-two cancer nodules (94%) were characterized by irregular edge visible burr, 26 (76%) can be seen with "needlepoint"microcalcification, and 6 (20%) showed neck metastasis lymph nodes, in which 2 appeared "needlepoint" microcalcification within lymph nodes. Conclusion Ultrasound of TMC has certain specific structural characteristics, with burr and "needlepoint" microcalcification, which provides important value for the diagnosis of TMC.
