摘要
目的：了解我院门诊处方及住院医嘱抗菌药物的应用情况，为临床合理用药提供依据。方法：对2012年我院门诊处方及住院医嘱基本情况以及抗菌药物的使用情况采用Excel进行数据统计分析，以抗菌药物使用类别及品种排序为指标对我院门诊及住院患者抗菌药物使用情况进行全面评价。结果：2012年我院门诊各季度抗菌药物的使用率在9．15％～11．37％之间，全年抗菌药物使用率为10．24％，抗菌药物处方平均品种数为1．24，抗菌药物联合用药占处方比例为21．37％，各季度基本持衡，我院门诊患者使用抗菌药物以口服为主。病区抗菌药物使用频率和使用强度皆为40％左右；住院患者使用抗菌药物以注射剂为主，主要用于外科手术预防用药。结论：我院门诊处方及病区医嘱抗菌药物应用基本合理，但存在抗菌药物使用中治疗方案不合理及无指征使用抗菌药物等问题。医院应加强医院药学建设，充分发挥医院临床药师的监测、监督与协助作用，促进临床科学、合理地使用抗菌药物。
OBJECTIVE： To investigate the application of antibiotics in outpatient and inpatient prescriptions of our hospital, and to provide evidence for rational drug use in the clinic. METHODS： General information and the application of antibiotics in out- patient and inpatient prescriptions of our hospital in 2012 were analyzed statistically. The application of antibiotics in outpatients and inpatients of our hospital was evaluated comprehensively using types and categories of antibiotics. RESULTS ： The utilization ra- tio of antibiotics in outpatient department of our hospital in 2012 were 9.15% -11.37%, and that of antibiotics was 10.24% yearly; there were 1.24 kinds of antibacterial drug on the average, and combined use of antibacterial drugs accounted for 21.37% and kept balance basically in each quarter; the outpatients of our hospital mainly were given antibacterial drugs orally. DDDs and AUD of an- tibiotics in wards was about 40% ; the inpatients mainly were given injection for prophylactic drug use in surgical operation. CON- CLUSIONS ： The utilization of antibiotics in outpatient prescriptions and inpatient prescriptions is reasonable in our hospital basical- ly, but there are some problems as unreasonable antibiotics scheme and no indications for use of antibiotics. The hospital should strengthen the construction of hospital pharmacy, give full play the role of monitoring, supervision and assistance to promote scientif- ic and reasonable use of antibiotics.
