摘要
目的观察下肢骨折术后早期持续被动活动功能锻炼对预防深静脉血栓形成的有效性。方法选择下肢骨折手术患者60例,随机分为治疗组和对照组各30例,术前彩色超声多普勒证实双下肢无深静脉血栓形成。手术后对照组常规预防性使用抗生素,行下肢肌肉的等张收缩以及踝关节和趾间关节主动屈、伸练习;治疗组在对照组基础上,手术后第2日应用持续被动锻炼(CPM)治疗仪,观察血栓发生率。结果治疗组无1例形成深静脉血栓,对照组深静脉血栓形成6例。治疗组明显优于对照组(P<0.05)。结论下肢骨折术后早期进行持续被动结合间歇主动规律的功能锻炼,可有效预防深静脉血栓形成。
Objective: To explore the validity of lower extremity fracture patients early postoperative Continuous Passive Motion (CPM)in the prevention of deep vein thrombosis (DVT). Methods: Sixty patients undergoing or- thopedic surgery were randomly divided into treatment group and control group. 30 patients in each group were examined with color doppler imaging. It was confirmed that these patients weren't suffered from DVT before surgery. After surgery, antibiotics was applied and both isotonic contraction of lower limb muscles and active flex- ing and stretching exercises of ankle and toe joints were done in control group. Besides control group's therapy, CPM was used in treatment group the next day after the operation. The incidence of thrombosis was recorded in each group. Results: None of patients in treatment group suffered from deep vein thrombosis,while 6 patients suffered from that in control group. The treatment group was better than control group (P 〈 0.05). Conclusion: Continuous passive moving of lower limb combined with intermittent active regular functional exercise can be ef- fective in preventing deep vein thrombosis of early postoperative lower limb fracture patients.
