摘要
目的探讨联合应用利多卡因皮内阻滞与辣椒碱软膏治疗神经性皮炎临床疗效,以及对患者局部皮损部位P物质表达情况的影响。方法局限性神经性皮炎患者81例,随机分为对照组(n=40)和治疗组(n=41),治疗组给予利多卡因皮内阻滞联合辣椒碱软膏治疗,对照组只进行皮内注射利多卡因治疗。治疗结束后1个月观察疗效及不良反应,电话随访3个月内的复发情况,记录患者治疗前后症状、体征评分情况及总有效率。结果治疗组总有效率显著高于对照组,差异有统计学意义(χ2=7.73,P<0.05)。对照组治疗后症状缓解平均时间(7.3±1.2)d,低于治疗组的(2.8±1.8)d,2组相比差异有统计学意义(P<0.05)。治疗后2组皮损组织中P物质水平均降低,治疗组降低明显,与对照组相比差异有统计学意义(P<0.05)。治疗组痊愈35例3个月后均得到随访,复发1例,在原发处复发。对照组痊愈23例中4例复发,3例在原发部位复发,1例在新部位复发。治疗组复发率低于对照组,2组比较差异有统计学意义(χ2=4.43,P<0.05)。结论联合应用利多卡因皮内阻滞与辣椒碱软膏治疗可明显改善神经性皮炎患者的临床症状,治疗起效时间短,复发率低,值得临床推广应用。
Objective To study the clinical efficacy of lidocaine injection comblned with capsacin oinunent u neurodermatitis, as well as its effects on substance P expression. Methods Eighty-one patients with neurodermatitis were randomly divided into control group （n= 40） and treatment group （n= 41）. Patients in treatment group were treated with lidocaine injectioncombined with capsaicin ointment and control group were only treated with lidocaine injection. At 1 month after treatment adverse effects were observed, recurrence within 3 months were followed up, symptoms and signs score were recorded before and after treatment, and the total efficiency of treatment were calculated. Results The total efficiency of treatment group was significantly higher than that of the control group （x2= 7.73, P〈0.05）. The relief time of symptoms in control group were （7.3 ± 1.2） days, significantly higher than （2.8 ± 1.8） days in treatment group （P〈0.05）. After treatment, the levels of substance P in lesions of patients were reduced in two groups, while that in treatment group were significantly lower compared with that of control group （P〈0.05）. After three months follow-up, 35 cases in the treatment group were cured, 1 case relapsed, recurrence at the primary location. 23 cases in control group had recovered, while 4 cases had recurrence, 3 cases re- curred at the primary site, 1 case at the new site. The difference of recurrence rate wa;s statistically significant （X2 = 4.43, P〈0.05）. Conclusion Lidocaine injection combined with capsaicin ointment can significantly improve the clinical symptoms of neurodermatitis, shorten onset time, with low recurrent rate, and is worthy of clinical application.
