22例乳腺癌二维及多普勒诊断分析

Analysis of Two-dimensional and Doppler Ultrasound Diagnosis of 22 Cases of Malignant Breast Cancers

摘要 目的:探讨乳腺癌的超声特点。方法:经二维超声了解22例乳腺癌的形态、边界、边缘、纵横比、内部回声、有无后方衰减及侧方声影、有无对周围组织的浸润等,然后用彩色多普勒检查,观察肿物内部及周边有无血流,频谱多普勒阻力指数高低进行分析。结果:本组绝大多数乳腺癌形态不规则(95.4%)、边界不清晰(86.3%)、内部回声不均匀(90.9%)、纵横比>1(95.4%)、对周围组织浸润(68.2%)、多结节状(68.2%)、肿物血流显示率(90.9%)、阻力指数以大于0.7(100%)为主。根据以上结果得出,只要肿物具备:(1)形态不规则;(2)边界不清晰;(3)边缘不规则;(4)内部回声不均匀;(5)纵横比>1;(6)周围组织浸润;(7)血管阻力指数>0.7;以上7项中的4项,即可考虑为恶性肿物,其准确性达75%。结论:乳腺癌的超声诊断应以二维声像图为基础,同时密切结合肿物的多普勒血流信号及阻力指数高低。 Objective:To analyze the performances of two -dimensional and color Doppler ultrasound of breast cancer , and to investi-gate the ultrasonic characteristics of breast cancer .Methods:22 cases of breast cancer were studied by two -dimensional ultrasound to de-tect the forms, borders, edges, aspect ratio, internal echo, with the rear side of the ultrasound attenuation and shadow or not , with infil-tration of the surrounding tissue or not .Furthermore, color and spectrum Doppler ultrasound were utilized to check whether the mass had internal and peripheral blood flow , and to measure the resistance indices .Results:The vast majority of the breast cancer were with irregu-lar shape (95.4%) , unclear boundaries ( 86.3%), uneven internal echo ( 90.9%) , aspect ratio 〉1 ( 95.4%) , invasion of surrounding tissue ( 68.2%) , multi-nodular shape ( 68.2%) , manifestation of tumor blood flow ( 90.9%) , and resistance index more than 0.7 ( 100%).From the results obtained , as long as the masses have more than four of the following seven features ,①irreg-ular shape,②unclear boundary,③irregular edges,④heterogeneous internal echo ,⑤aspect ratio 〉1;⑥surrounding tissue infiltra-tion ,⑦vascular resistance index 〉0.7, can be considered as malignant tumors .The accuracy was 75%.Conclusions:The ultrasound diagnosis of breast cancer should be based on two -dimensional ultrasound , while combined with the Doppler flow signals and resistance index level closely .
