摘要
目的 探讨超敏肌钙蛋白T(hs-cTnT)对于鉴别诊断急性心肌梗死(AMI)及心肌损伤的应用价值.方法 选择2011年8月至2013年5月深圳市龙岗区第二人民医院收治的AMI及非心脏病患者255例为研究对象,采用电化学发光免疫分析法检测hs-cTnT表达水平,依据其水平分为低于下限组(≤3 μg/L)、中间组(3~14 μg/L)、增高组(≥14 μg/L),分析超敏心肌钙表达水平的决定因素,探讨其对心肌损伤的预测价值.结果 255例患者中,134例(52.55%)可以检测到hs-cTnT≥3 μg/L,28例(10.98%)可以检测到hs-cTnT≥14 μg/L;超敏心肌钙表达水平与年龄、男性、脑钠肽前体、空腹血糖呈正相关性,与肾小球滤过率呈负相关性(P<0.05);中间组、增高组AMI及心肌损伤10年发生风险显著高于低于下限组(OR=3.232,95% CI 2.254~4.653,P<0.01;OR=2.184,95% CI 1.685~4.654,P<0.01).结论 hs-cTnT≥3 μg/L、≥14 μg/L可能为鉴别诊断心肌损伤与 AMI的两个决定水平,对于预测及预后有积极的应用价值.
Objective To study application value of high-sensitivity cardiac troponin T (hs-cTnT) dif- ferential diagnosis of AMI and myocardial injury. Methods A total of 255 acute myocardial infarction and non-heart disease patients were chosen as the objects, electrochemiluminescence immunoassay was done for detection of hs-cTnT expression levels, based on which the objects were divided into below the lower limit group( ≤3 μg/L) ,intermediate group(3-14 μg/L) ,increase group( ≥14 μg/L) ,hs-cTuT expression level decisive factors were analyzed to explore the predictive value for myocardial injury. Results Among the 255 patients,hs-cTnT≥ 0. 003 μg/L was detected in 134 cases(52.55% ) ,hs-cTnT≥14 μg/L was detected in 28 cases ( 10.98 % ) ; hs-cTnT expression level and age, male, brain natriuretic peptide precursor, and fasting plasma glucose were positively correlated, and negatively correlated with glomerular filtration rate(P 〈 0.05 ). The AMI and myocardial injury in 10 years of the intermediate group and increase group were significantly higher than that of below the lower limit group ( OR = 3. 232,95 % CI 2. 254-4. 653, P 〈 0.01 ; OR = 2.184, 95% CI 1. 685-4. 654 ,P 〈0.01 ). Conclusion hs-cTnT≥3 μg/L, ≥ 14 μg/L might be the two decisive levels for differential diagnosis of myocardial injury and the acute myocardial infarction ,which have applica- tion value for the prediction and prognosis.
