摘要 目的探讨在非糖尿病的高血压人群中糖化血红蛋白与心血管重构的关系。方法对150例原发性高血压但未诊断糖尿病的住院患者检测总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甘油三脂(TG)、空腹血糖(FPG)水平,同时检测糖化血红蛋白(HbA1c)水平。以30例健康人血标本为健康对照组,检测同样项目并与高血压组进行对比分析。结果 (1)高血压组HbA1c显著高于正常组(P<0.05);(2)高血压组患者LVMI及IMT/D显著高于正常组(P<0.01)。随血压水平升高,HBAc、IMT/D、LVMI呈上升趋势,高血压1级、2级组与高血压3级组组间比较有统计学差异(P<0.01);(3)相关析表明高血压组患者的HbA1c水平与LVMI、IMT/D呈明显相关性(P<0.01);(4)对影响IMT/D及LVMI因素的多元线性回归分析显示,HBA1c是其主要影响因素。结论 HbA1c水平是非糖尿病原发性高血压患者心血管重构发生的危险因素之一。 Objective To investigate the relationship between glycosylated hemoglobin (HbAIe) and cardiovascular remodeling in patients with non - diabetic hypertension. Method 150 eases of hospitalized patients with primary hypertension but not with diagnosed diabetes were detected in terms of tile levels of total cholesterol (TC), high density lipoprotein cholesterol (HDL- C), low density lipo- protein cholesterol (LDL- C), glycerin three fat (TG), fasting blood g!ueose (FPG), as well as glycosylated hemoglobin (HbAle) at the same time. Tile same indicators of the control group wlfich was composed of 30 case healthy human blood specimens were tested and compared with those of hypertension group. Results Patients' HhAlc level in hypertension group was significantly higher than that in nomlal group ( P 〈 0. 05 ) , and patients' LVMI and IMT/D levels in hypertension group were significantly higher than those in normal group (P 〈 0. 01 ). With elevated blood pressure, HhAlc, LVMI artd IMT/D were on the rise. The comparisons between groups of high blood pressure level 1 & level 2 with group of high blood pressure level 3 was statistically significant ( P 〈 0. 01 ). Relevant analysis showed that in hypertension group, patients' HbAle level significantly related to LVMI and IMT/D (P 〈0. 01 ). The multiple linear re- gression analysis of factors affecting IMT/D and LVMI showed that HBAle was one of the main influencing factors. Conclusion HbAI c level is a risk factor for cardiovascular remodeling in patients with non -diabetic primary hypertension.
