摘要
目的:研究在门静脉高压症上消化道出血疾病治疗中采用贲门周围血管离断加胃底环行缝扎手术的治疗效果。方法:选取在笔者所在医院肝胆外科2009-2011年收治的患有肝硬化疾病患者中60例门静脉高压症合并上消化道出血的患者,随机分为对照组和观察组,对照组实行传统的门奇静脉断流术,观察组实行改良门奇静脉断流术。对两组患者住院时间、手术后并发症以及手术前后肝功能指标的改变对比分析。结果:观察组住院时间明显少于对照组;观察组术后再出血率少于对照组;两组术后并发症比较差异无统计学意义;两组手术前后肝功能指标均无明显变化。结论:在门静脉高压症合并上消化道出血治疗中实行门奇静脉断流改良术会取得更好的治疗效果,在临床治疗具有重要的意义,值得推行应用。
Objective：To study the portal hypertension hemorrhage of upper digestive tract diseases treated with pericardial devascularization plus gastric fundus suture operation treatment.Method：60 cases of portal hypertension with gastrointestinal bleeding in our hospital from 2009 to 2011 in department of hepatobiliary surgery which suffering from cirrhosis of the liver disease,60 patients were randomly divided into control group and observation group,30 patients used the traditional portal azygous disconnection,the observation group used with improvement of portal azygous disconnection,the length of stay,operation complications and changes of liver function before and after operation were compared and analyzed.Result：The length of stay in the observation group were significantly less than that of the control group.The incidence of bleeding in the observation group were significantly less than that of the control group.The operation complication was no significantly different between two groups of patients.The liver function had on change before and after operation treatment in two groups. Conclusion：Improved door azygos vein block can get better effect in the treatment of portal hypertension complicated with gastrointestinal bleeding,and it is important significance in clinical treatment.
