包膜挛缩的危险因素分析:一项SIENTRA圆形光面及毛面假体隆乳术的5年研究分析

背景与目的尽管有一些广泛认可的相关因素,但导致包膜挛缩的多层面的因素在近几十年内仍未解决。本研究将调查首次隆乳术患者导致包膜挛缩的一些潜在危险因素。方法本研究资料包含2560例首次隆乳术患者,假体5109只,由34位医师在5年内施行的SIENTRA临床研究结果。患者每年随访一次,记录包膜挛缩的Baker分级。潜在危险因素包括患者属性、假体属性、手术特征、腔隙冲洗和术后特征,对其进行频率和多变量模型分析。结果在5年中,共有179例患者的265只乳房假体发生包膜挛缩。由假体产生的包膜挛缩的总体Kaplan—Meier概率为7.6%。未调整的分析显示,以下因素会增加包膜挛缩的发生概率:光面假体、环乳晕切口、乳腺下层面置入、抗生素及类固醇腔隙冲洗、被建议按摩及外科胸罩(P值均〈0.05)。将所有危险因素调整为模型中的变量,其多变量分析的结果发现6种因素与包膜挛缩有独立关联(假体置入层次、假体表面、切口位置、血肿或血清肿发展、假体尺寸及外科胸罩;P值均〈0.05)。结论本研究分析证实,排除其他因素的情况下,肌肉下层次置人和选择毛面假体可显著减少包膜挛缩的发生。
