摘要
目的探讨硬膜外镇痛在高龄患者前列腺电切术后的应用效果。方法选择医院自2012年7月至2013年6月收治的65例择期行经尿道前列腺电切术患者的临床资料．根据随机原则将患者分为观察组33例与对照组32例。两组患者术中麻醉方式相同，术后观察组患者给予硬膜外镇痛．对照组患者给予静脉镇痛。结果全部患者均顺利完成手术，术中两组患者在血压、心率变化无显著性差异。两组患者术后24hVAS评分对比具有显著性差异（P〈0．05）；术后两组发生膀胱痉挛比率对比，结果具有显著性差异（P〈0．05）；两组患者停止冲洗时间、平均住院时间对比，结果具有显著性差异（P〈0，05）。观察组患者麻醉前、后5min与30min血压、心率变化情况对比，无显著性差异伊〉0．05），临床疗效较为满意；术后患者血肌酐与血尿素氮均有所上升，但与术前对比无统计学差异（P〉0．05）。结论硬膜外镇痛具有起效快、麻醉效果完善、安全性高等优势，较适用于高龄患者前列腺电切术后镇痛，可临床推广。
Objective to study the epidural postoperative analgesia in elderly patients with prostate electricity cut the clinical ap- plication.Methods our hospital from July 2012 to June 2013 treated with transurethral plasmakentic vaporization of prostate of 65 patients undergoing elective electricity cut method the clinical data of patients,based on the principle of random,33 cases of pa- tients were divided into observation group and control group in 32 cases,two groups of patients with same way,intraoperative anesthesia in observation group patients give epidural analgesia,control group patients were given intravenous analgesia.Results all patients were successfully completed surgery,patients in two groups there was no significant difference in blood pressure,heart rate changes.Two groups of patients with postoperative 24 hvas score comparison with significant difference （P 〈 0.05）,two groups of postoperative bladder spasm contrast ratio,the result has a significant difference （P 〈 0.05）,two groups of patients stop flush time, average length of hospital stay,the resuh has a significant difference（P 〈 O.05）.Observe anesthesia group patients before and after 5 min compared with 30 min,the change of the blood pressure,heart rate,there was no significant difference （P 〉 0.05）,and ob- tained satisfactory clinical effect.postoperative serum creatinine and blood urea nitrogen were increased,but the comparison,no sig- nificant difference（P 〉 0.05）.Conclusion has quick effect,anesthesia epidnral analgesia effect,high security advantages,more suit- able for senile patients with prostate electricity cut postoperative analgesia,clinical application and dissemination.
