摘要
目的:提高X线与CT检查对骨纤维异常增殖症(fibrous dysplasia of bone,FDB)的认识,探讨其影像学表现与病理之间的关系。方法:回顾分析17例由手术病理证实的FDB的X线与CT表现。结果:由于病理上骨组织成分的不同,FDB在X线片和CT上表现为皮质变薄,呈膨胀性骨质破坏,无骨膜反应及软组织肿块5例;髓腔呈磨玻璃样密度改变3例;骨干增粗、变形,髓腔呈丝瓜肉样改变4例;虫蚀样改变1例,骨质广泛增生改变2例;有上述两种或两种以上改变2例。结论:大多FDB具有较典型的影像学表现,X线片大多可显示病变的整体观及病变特点,但CT可通过横断面及多平面图像重组来显示病变的内部结构、比邻关系、范围,同时可测量CT值,尤其在解剖结构复杂的部位更具优势,可作为首选检查。
Objective:Further study the X-ray and CT examinations on fibrous dysplasia of bone(FDB) and discuss the relationship between its imaging expressions and pathology. Methods: Retrospectively analyze the X-ray and CT expressions of 17 cases of FDB proved by operational pathology. Results:Because of the different pathological compositions of bone tissues,the X-ray and CT expres-sions of FDB are :5 cases of soft tissue swelling with thinner cortex, expansive destructions of bone and without periostea] reaction;3 ca-ses with ground glass opacity density changes in pulp cavity;4 cases with thickening and deformation of backbone and luffa-shape chan- ges in pulp cavity;1 case of wormy appearance changes ;2 cases with wide range of hyperostosis. There are 2 cases including two or more than two changes mentioned above. Conclusion : Most FDB have typical imaging expressions. The X-ray showed the holism and charac-teristics of lesion while the CT showed the inner structure, neighboring relationship and ranges of the lesion by cross sectional and multi-plane images reorganization. Meanwhile,the data of CT can be tested,which shows priority at the complex anatomical positions. So it is the first choice of examination.
