摘要 目的了解本院医院感染的现状,为提高医院感染的管理水平提供科学依据。方法采用横断面调查、床旁调查和病历调查相结合的方法,对住院患者进行调查,各科室监控人员使用与医院信息系统兼容的现患率调查软件,现场录入现患率调查资料,院感科在此基础上进行统计、生成有关数据、分析。结果三年医院感染平均实查率为99.35%,医院感染例次发病率分别为4.50%、3.29%,2.64%;感染率最高的科室是重症监护病房40%、50%、37.5%,医院感染高发部位以下呼吸道感染居首位,占41.82%;抗生素使用率分别为37.66%、35.66%、27.00%;根据药敏使用抗生素率为41.73%、37.27%、57.27%。结论有针对性地对重点科室、重点环节、高危人群加强目标性监测,加强对非医院感染病人使用抗菌药物前标本送检率监控,重视抗生素的合理应用,特别是围手术期患者抗生素的应用,可有效地控制医院感染的发生;信息化的现患率调查快捷、准确、效率高、效果好,有推广价值。 Objective To learn the current status of the hospital infection so as to provide a scientific basis for improving the management of nosocomial infection. Methods By means of combining a cross - sectional survey with the bedside investigation and review of medical records, the hospitalized patients was investigated. Monitors of every department used a software of investigation of prevalence rate compatible with hospital information system (HIS) to input the data of investigation of prevalence rate on the scene, then department of hospital infection management generated and statistically analyzed the related data by the software. Results The average actually investigated rate of nosocomial infection was 99. 35% in three years. The case - time incidence of nosocomial infection were respectively 4. 50%, 3.29% and 2. 64%. ICU was with the highest infection rate, accounting for 40% ,50% and 37. 5% ; The sites at high -risk of nosocomial infection was the lower respiratory tract, accounting for 41.82% ;the utilization rate of antibiotics were 37.66% ,35.66% and 27.00% ; the application rates of antibiotics by drug - sensitive test were 41.73% ,37. 27%, 57.27%. Conclusion On the one hand, the prevalence rate survey will enhance targeted sur- veillance to the key departments, the key links and high risk populations, and strengthen the monitoring of specimens submission rate of un - hospital infection patients before using antibiotics, in the meantime, regulate the rational use of antibiotics, only we do all those can the occurrence of nosocomial infection be controlled effectively; On the other hand, the investigation of information to prevalence rate is convenient, accurate, efficient and effective, it is worth popularizing.
