摘要
目的:探讨宫颈脱落细胞中人乳头瘤状病毒(humanpapillomavirus,HPV)16型整合状态及病毒载量在宫颈病变中的意义。方法:应用多聚酶链反应(polymerasechainreaction,PCR)及实时定量PCR扩增,检测HPV16E2、E6及人看家基因β-actin的拷贝数,分析HPVl6病毒载量和整合状态与宫颈病变的关系。结果:428例标本中,检测到27例HPV16阳性,其病毒载量介于2.8~118.9拷贝/细胞;其中4例(14.8%)E2/E6比率〉1,为纯游离型:6例(22.2%)E2/E6比率=0,为纯整合型;17例(63.O%)E2/E6比率〉0而〈1,为整合与游离的混合型:并且随病变程度加重,游离型比例逐渐降低,整合型(游离+整合)比例逐渐升高。结论:HPV16整合入宿主基因组在部分宫颈上皮病变中已经存在:检测HPV16E2、E6及其比值,可以作为宫颈病变早期筛查的一种手段.以便及早发现宫颈病变的高危患者。
Objective To investigate the significance of the integration status and viral load of human papillomavirus(HPV)16 in cervical exfoliated cells. Methods The copy number of HPV16 E2, E6 and human housekeeping gene β-actin were detected by polymerase chain reaction (PCR) and quantitative real-time PCR amplification for the analysis of the relationship of HPV16 viral load and integration status with cervical lesions. Results HPV was positive in 27 out of 428 specimens, and the viral load of HPV16 was ranged from 2.8 to 118.9 copies / cell; Among them, 4 cases (14.8%) had the E2/E6 ratios 〉 1 and was classified as the episomal form, 6 cases (22.2%) had the E2/E6 ratios = 0 as the integrated form, and 17 cases (63.0%) had the E2/E6 ratios from 0 to 1 as the mixture of episomal and integrated form. The prevalence of episomal form was decreased gradually, while those of integrated form and mixture form were increased with the progression of cervical disease. Conclusion Integration of HPV16 into the host genome was already present in some cervical intraepithelial lesions. The detection of HPV16 E2, E6 genes and E2/E6 ratio could be used as a tool for early screening of high-risk patients with cervical lesions
