摘要
目的 观察天冬氨酸特异性半胱氨酸蛋白酶-3(Caspase-3)在重症急性胰腺炎(SAP)大鼠胰岛β细胞损伤中的作用.方法 雄性Wistar大鼠24只,体质量200-250 9,随机分为两组:假手术组(SO组)和重症急性胰腺炎模型组(SAP组).大鼠禁食12h后采用胆胰管逆行注射5%牛磺胆酸钠制备SAP模型,造模后6h取血测定空腹血糖和血清胰岛素水平;随后腹腔注射20%葡萄糖液(2 g/kg),30 min后再次抽血测定血糖和血清胰岛素水平.取胰尾部胰腺组织,电镜下观察大鼠胰岛β细胞超微结构变化,免疫组织化学分析胰岛β细胞Caspase-3表达.结果 两组空腹血糖差异无统计学意义(P>0.05),但是SAP组大鼠糖负荷后血糖明显高于SO组(P<0.05).糖负荷后,SO组大鼠血清胰岛素明显升高,而SAP组有所下降,两组差异有统计学意义(P<0.05);电镜下SAP组大鼠胰岛β细胞出现核固缩、染色质边集,线粒体、内质网、高尔基复合体肿胀,分泌颗粒减少等一系列超微结构病理改变,SAP组Caspase-3的表达较SO组增强,差异有统计学意义(P<0.05).结论 SAP大鼠血糖调控能力降低可能与胰岛β细胞Caspase-3表达增多有关,导致其超微结构的改变,从而影响胰岛素的分泌.
Objective To explore the effect of cysteinyl aspartate specific proteinase-3 (Caspase-3)on pancreatic beta cell damage in severe acute pancreatitis (SAP).Methods Twenty-four male Wistar rats weighing 200-250 g were selected and randomly (random number) divided into the sham operation group (SO group) and SAP group (n =12 each).The SAP model was induced by retrograde injection of 5% sodium taurocholate into the biliopancreatic duct.Rats were killed at 6 h after operation.The serum glucose (GLU) and plasma insulin (INS) levels were measured.After intraperitoneal injection of 20% glucose (2 g/kg) (a glucose load),GLU and INS were measured 30 min later.The ultrastructure of the pancreatic beta cells was observed with an electron microscope.The expression of Caspase-3 was detected immunohistochemically.Results The serum glucose level 30 min after a glucose load and the expression of Caspase-3 were increased significantly in the SAP group as compared with the SO group (P 〈0.05).The plasma insulin level was increased significantly 30 min after a glucose load in the SO group as compared with the SAP group (P 〈 0.05).Pathological uhrastructural changes were found as follows:most pancreatic beta cells in the SAP group exhibited vacuole degeneration,the mitochondria had swolen and the rough endoplagmic reficuhm had expanded,and Caspase-3 expression was increased in the SAP group as compared with the SO group (P 〈 0.05).Conclusion The increased beta cell Caspase-3 expression may reduce the blood glucose control ability of pancreatic beta cells in the SAP rats,leading to the ultrastructural changes,thus affecting the secretion of insulin.
