四磨汤对便秘型肠易激综合征模型大鼠的干预作用 被引量:5

Intervention Effect of Simo Decoction on Rats with Constipation-predominant Irritable Bowel Syndrome

摘要 【目的】观察四磨汤对便秘型肠易激综合征(IBS-C)的干预作用及可能的机制。【方法】选用SPF级SD大鼠,将其随机分为正常组,模型组,中药高、中、低剂量组(剂量分别为3.6、1.8、0.9 g·kg-1·d-1),阳性对照组(马来酸曲美布汀,剂量为0.1 g·kg-1·d-1);采用夹尾激怒联合直肠刺激法复制大鼠IBS-C模型,给药后测定各组大鼠下丘脑、脑干部位的肾上腺皮质激素释放因子(corticotropin-releasing hormone,CRH)水平,腰段脊髓P物质(substance P,SP)含量,肠黏膜肥大细胞活化状态(c-fos基因表达情况)。【结果】经四磨汤治疗2周后,与模型组比较,中药高、中、低剂量组大鼠内脏高敏感性均显著降低(P<0.05)。与正常组比较,模型组大鼠脑干和下丘脑部位的CRH含量显著升高,脊髓SP含量显著降低,肠黏膜c-fos基因表达总阳性显色细胞数显著降低,差异均有统计学意义(P<0.05);与模型组比较,阳性对照组,中药高、中、低剂量组大鼠脑干和下丘脑CRH水平均显著降低,大鼠脊髓SP含量显著升高,肠黏膜c-fos基因表达总阳性显色细胞数显著升高,差异均有统计学意义(P<0.05)。【结论】四磨汤治疗IBS-C的作用与其降低大鼠脑干、下丘脑CRH含量,升高脊髓P物质含量与c-fos基因表达水平,影响脑—肠轴通路有关。 Objective To investigate the intervention effect and the possible therapeutic mechanism of Simo Decoction (SD) for constipation-predominant irritable bowel syndrome (IBS-C). Methods SD rats were randomly divided into 6 groups, normal group, model group, western medicine control group (treated with Trimebutine 0.1 g·kg^-1·d^-1), and low-, middle- and high-dose SD groups (in the dosage of 3.6, 1.8, 0.9 g·kg^-1·d^-1, respectively). IBS-C model was established by tail-clamping method combined with rectal stimulation method. The content of corticotropin-releasing hormone (CRH) in the hypothalamus and brainstem, substance P (SP) in lumbar spinal cord, C-fos mRNA expression in activated distal colonic mucosal mast cells of all the groups were observed. Results On week 2 after treatment with SD, visceral hypersensitivity in low-, middle- and high-dose SD groups was significantly decreased (P〈0.05 compared with that in the model group). Compared with the normal group, CRH content was increased (P〈0.05) , SP content and the count of distal colonic mucosal mast cells with positive c-fos mRNA expression were significantly decreased (P〈 0.05) in the model group. Compared with the model group, CRH content was decreased (P〈0.05), and SP content and the count of distal colonic mucosal mast cells with positive e-los mRNA expression were significantly increased (P〈 0.05) in the four medication groups. Conclusion The therapeutie mechanism of Simo Decoction for IBS-C is probably related with the decrease of CRH content spinal SP content and c-fos mRNA expression. in the hypothalamus and brainstem, and with the increase of
