摘要
目的探讨铁皮石斛辅助治疗HBeAg阳性慢性乙肝患者临床效果及其与细胞因子变化的关系。方法对入选患者随机分为两组,113例慢性乙肝患者(治疗组)在常规治疗的基础上,用铁皮石斛处方,连用6月,观察ALT正常率、HBV DNA转阴率和HBeAg血清学转换率、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)和白细胞介素-10(IL-10)水平,并与110例常规治疗组(对照组)和30例健康组比较。结果治疗组HBV DNA转阴率和HBeAg血清学转换率分别为38.1%和19.5%,较对照组24.5%和10.0%高(P<0.05),治疗组ALT正常率98.2%与对照组97.3%比较差异无统计学意义(P<0.05)。治疗前两组IL-2水平低于健康组(P<0.01),IL-4和IL-10水平高于健康组(P<0.01),治疗后治疗组IL-2、IL-4和IL-10与健康组比较差异无统计学意义(P>0.05),对照组IL-2、IL-4和IL-10水平均有好转,但与健康组比较差异仍有统计学意义(P<0.01)。结论铁皮石斛可提高HBeAg阳性慢性乙肝患者的ALT正常率、HBV DNA转阴率和HBeAg血清学转换率,有效改善患者的IL-2、IL-4和IL-10紊乱,对减轻症状、减少并发症的发生、促进患者康复等有辅助治疗作用。
Objective Discuss Dendrobium officinale Kimura et Migo in the adjuvant treatment of HBeAg positive chronic hepatitis g patients,focusing on clinical effects and the change of cytokines. Methods 113 cases of chronic hepatitis B patients on the basis of routine therapy, combined with Dendrobium officinale Kimura et Migo prescription for 6 months. The serum ALT, HBV DNA turn negative rate and HBeAg conversion rate,interleukin-2 （IL-2） and interleukin-4 （IL-4） and interleukin-10 （ IL 10） levels were determined. The results were compared to 110 cases with conventional treatment and 30 cases of healthy group. Results In the treatment group HBV DNA turn negative rate and HBeAg conversion rates were 38.1% and 19.5 ~ respectively, higher than control group of 24.5 and 10.0~ ,respectively（P〈~0.05）. In treatment group ALT normal rate was 98.2~ compared with 97.30/00 in the control group（P^0.05）. Before treatment levels of IL-2 were lower in HVB groups than in healthy group（P^0. 01）, while IL-4 and IL-10 levels were higher than healthy group（P^0.01）. After treatment the levels of IL-2,IL-4 and IL 10 were not statistically different from healthy group（P^0.05）, in the control group IL-2, IL-4 and IL-10 levels were improved,but still different from healthy group（P^0.01）. Conclusion Dendrobium o{ficinale Kimura et Migo can improve HBeAg positive chronic hepatitis B patients on ALT, HBV DNA turn negative rate and HBeAg conversion rate,and effectively return serum IL-2,IL-4 and IL 10 levels to normal. Thus,Dendrobium is effective in the treatment of hepatitis B patients.
