防旋加压髓内针在股骨干中下段骨折中的应用 被引量:1

摘要 目的 :测试作者研制的防旋加压髓内针 (antirotatedcompressiveintramedullarynail,ACIN)的生物力学性能及探讨其在股骨干中下段骨折的治疗中的临床应用。方法 :将 14根成人股骨标本分成两组 ,制成中下 1/3骨折模型 ,一组置入ACIN及梅花针 ,另一组置入双Ender钉 ,在SWD 10实验机上进行非破坏性的三点弯曲实验 ,在MTS实验机上进行破坏性的扭转实验。临床应用 18例 ,包括股骨干中下段的横形、斜形、螺旋形及粉碎性骨折。结果 :ACIN组的最大扭距和弯曲刚度明显优于梅花针组和Ender钉组 (P <0 .0 1)。全部病例均获随访 ,随访时间 9~ 2 7个月 (平均 14 .5个月 ) ,骨折全部愈合。按照Klemm功能恢复分级标准 ,总优良率 10 0 %。结论 :( 1)ACIN设计合理 ,其防止远侧骨折段旋转及抗弯曲的能力能够满足临床需要 ,可以用于治疗股骨干中下段骨折。 ( 2 )手术操作简单 ,术中不需要X线机监视 ,便于推广。 ( 3 )ACIN有一定的加压作用 ,有利于骨折的愈合。 Objective: To test the biomechanical characteristics of the antirotated compressive intramedullary nail(ACIN)designed by the authors and studied the use of it in the treatment of median and lower part femoral shaft fractures.Methods:Fourteen specimens of femur undergone total osteotomy at median and lower third position were divided into two groups.One group was implanted with Kuntscher's nail first then changed into ACIN.Another group was implanted with two Ender's nails only.Non distory bending test was performed on the SWD 10 testing machine and distory torsion test was performed on the MTS testing machine.Eighteen cases of median and lower part of femoral shaft fractures were treated with open nailing procedure including transverse,oblique,helicoids and comminute fractures.Result:All results suggested that the torsion rigidity and bending stiffness of ACIN are much better than Kuntschert's nail and Ender's nail group (P<0.01).After 9 to 27 months (average 14.5 months) follow up all of the fractures had union and the overall excellent rate was 100% according to Klemm grading.Conclusion:(1) ACIN is well designed.Its satisfy of preventing distal part of fracture from rotating and its bend stability are sufficient for clinical needs.It can be used in the treatment of median and lower part of femoral shaft fractures.(2) It is easy to use and X ray monitoring is not necessary during operation.(3) Its compressive effect help to promote the healing.
