超声下胆囊功能分级在微创保胆取石术中的应用价值分析 被引量:5

摘要 目的在超声动态监测下,通过胆囊脂餐试验,对胆囊结石患者进行胆囊排空功能的评价及分级,为临床选择纤维胆道镜微创保胆取石术(MC)或是经腹腔镜胆囊切除术(LC),甚至是开腹胆囊切除(OC)提供较为可靠的鉴别标准。方法选取我院胆囊结石伴慢性胆囊炎行MC及LC患者共275例,患者行常规脂餐试验,动态观察胆囊收缩情况,检测胆囊储存、收缩和释放胆汁的功能,并计算出排空率,将胆囊功能进行评分和分级,按胆囊收缩功能不同将其分为I~Ⅳ级。结果胆囊功能Ⅰ级80例,保胆成功率100%;胆囊功能Ⅱ级63例,保胆成功率95.24%;胆囊功能Ⅲ级34例,保胆成功率32.35%,无中转开腹;胆囊功能Ⅳ级98例,均行LC,保胆成功率0%。结论改良超声下胆囊功能的分级标准,给外科提供了一个较为可靠的标准,正确的选择的术式,不仅尊重了科学,也为患者提供了最佳个体化治疗方案,减少了并发症的发生。
