摘要
目的:研究慢性重型乙型肝炎患者外周血T细胞表面CD127表达水平的变化及与慢性重型乙型肝炎发病的关系。方法:流式细胞术检测30例慢性重型乙型肝炎、20例慢性乙型肝炎(CHB)患者和20例健康志愿者外周血T细胞中CD127的表达,用ELISA方法检测血清中细胞因子IL-7(白细胞介素-7)的表达。结果:慢性重型乙型肝炎组和CHB组CD8+T比例均较健康对照组高(P均<0.01),慢性重型乙型肝炎组CD4+T比例较健康对照组高(P<0.01)。慢性重型乙型肝炎与CHB组CD127+细胞的比例均较健康对照组低(P均<0.05)。慢性重型乙型肝炎组CD4+CD127+双阳性细胞的比例低于CHB组,CHB组又低于健康对照组(P均<0.01);慢性重型乙型肝炎组CD8+CD127+双阳性细胞比例较CHB组和健康对照组低(P均<0.01)。CD8+CD127+细胞的表达与HBV DNA载量呈负相关。慢性重型乙型肝炎组和CHB组血清IL-7含量均较健康对照组高(P均<0.01)。结论:慢性重型乙型肝炎患者外周血中CD4+CD127+、CD8+CD127+细胞表达降低;以CD8+CD127+降低更明显,并与HBV DNA载量呈负相关;血清中IL-7表达增高,可能与慢性重型乙型肝炎的发病有关。
Objective： To investigate the changes of CD127 expression in peripheral blood of patients with chronic severe hepatitis B and its correlation with the pathogenesis of chronic severe hepatitis B. Methods： Heparin antieoagulated venous bloods were obtained from 30 chronic severe hepatitis B patients, 20 patients with chronic B hepatitis （CHB） and 20 healthy volunteers （as controls） . CD127 expressions on CD4^ ＋ , CD8^ ＋T lymphocytes were examined by flow cytometry. Serum IL-7 levels were detected by ELISA. Results： The ratios of CD8 ^＋ T lymphocytes in CHB and chronic severe hepatitis B patients were higher than those in healthy eontrol（ P 〈 0. 01 ） , but only in chronic severe hepatitis B patients, the ratio of CD^＋ T lymphocytes was higher than in healthy control （P 〈0.01 ） . The ratios of CD127^ ＋ and CD4^ ＋ CD127 ^＋ lymphocytes in both chronic severe hepatitis B and CHB groups were lower than those in healthy group （P 〈 0. 05 and 〈 0. 01 ） . The ratio of CD8 ^＋ CD127^ lymphocytes in chronic severe hepatitis B patients was less than that in CHB and control groups （P 〈 0.01 ） . The expression of CD8 ^＋ CD127 ^＋ T was negatively correlated with HBV DNA level. The expression of IL-7 of healthy group was lower than CHB and chronic severe hepatitis B groups. Conclusion： CD127 as the marker of memory ceils has very important function in chronic severe hepatitis B, IL-7 is essential for the pathogenesis of severe hepatitis.
