摘要
目的观察补肾降浊方治疗慢性肾脏病(CKD)2-3期肾虚浊瘀证的临床疗效。方法采用前瞻性随机对照研究,将90例CKD 2-3期肾虚浊瘀证患者分为2组,对照组仅予基础治疗,治疗组予基础治疗联合补肾降浊方,疗程3个月。观察两组治疗前后中医证候积分24h尿蛋白定量、血肌酐、尿素氮、血清胱抑素、血清白蛋白、血红蛋白、C反应蛋白等变化情况,以及临床疗效和残存肾功能情况。结果治疗组、对照组总有效率分别为93.33%、73.33%,两组临床疗效差异有统计学意义(P<0.05)。两组治疗前后中医证候积分均较治疗前降低(P<0.05),治疗组低于对照组(P<0.05,P<0.01)。治疗组治疗后Scr、BUN、24h尿蛋白定量、血清胱抑素C和C反应蛋白显著低于对照组,Hb、Alb高于对照组(P<0.05,P<0.01)。结论补肾降浊方联合基础治疗能显著改善CKD2-3期肾虚浊瘀证患者的24h尿蛋白定量、血清胱抑素C和C反应蛋白水平,降低Scr、BUN水平,改善肾功能,提高患者的Hb、Alb,改善贫血和肾性营养不良,延缓CKD 2-3期发展至终末期肾衰的进程。
Objective To observe the clinical effieaey of ＂ Bushen Jiangzhuo Decoction＂ in treating chronic kidney disease （CKD） at 2-3 phase of kidney deficiency and blood stasis pattern. Methods Ninty eases with CKD at 2-3 phase were divided into two groups by prospective randomized controlled trial method. The eases in control group were treated with routine treatment, and cases in treatment group were treated with ＂Bushen Jiangzhuo Decoction＂ additionally, with the course of 3 months. The 24 hour urine protein, Ser, BUN, Cys, Alb, HB, CRP clinical efficacy and residual renal function were analyzed. Results The overall effeetive rate was 93.33% in the treatment group and 73.33% in the control group, with a dil＇t〉renee between tile two groups （P 〈 0.05 ）. The clinieal symptom scores were decreased after treatment in both of the groups （ P 〈 0.05 ）, the clinieal symptoms were impruved mueh more in treatment group than that of the eontrul group （ P 〈 0.01, P 〈 0.05 ）. The contents of Scr, BUN, 24 hour urine protein, CYS-C and CRP in treatment gruup were lower than that of the control group, while the Hb and Atb were higher than that of the control group （P 〈 0.05, P 〈 0.01 ）. Conclusion Comhined ＂ Bushen Jiangzhuo Decoction＂ and routine treatment can improve the levels of 24 h urine protein quantitation, CYS-C and CRP significantly. It can also decrease the Scr and BUN, improve the kidney function, Hb, Alb, anemia, renal osteodystrophy and retard the developing process of CKD 2-3 phase to end-stage renal failure.
