摘要
目的探讨不同海拔环境对慢性阻塞性肺疾病急性加重期(AECOPD)患者治疗前后血清中碱性成纤维细胞生长因子(bFGF)表达的影响和意义,及血清中bFGF的变化情况与FEV1/FVC%、FEV1%Pred的相互关系。方法采用酶联免疫吸附测定(ELISA)法测定两个地区AECOPD患者及健康对照组血清bFGF的含量,并行肺功能检查,统计分析检测数据。结果①两个地区AECOPD患者治疗前后血清bFGF水平均高于健康对照组,AECOPD患者治疗后bFGF水平较治疗前下降,且平原地区AECOPD患者治疗后血清bFGF下降程度稍高于中度海拔地区;平原地区健康对照组血清bFGF含量稍低于中度海拔地区健康对照组,差异有统计学意义(P<0.05);②分别比较两个地区AECOPD患者治疗前后血清bFGF含量,差异无统计学意义;③两个地区AECOPD患者治疗前后FEV1/FVC%、FEV1%Pred均明显低于对照组,治疗后FEV1/FVC%、FEV1%Pred较治疗前有所升高,差异有统计学意义(P<0.05);两个地区各组肺功能观察指标结果比较无统计学意义;④AECOPD患者血清中bFGF表达水平与FEV1/FVC%、FEV1%Pred无相关性。结论①两个地区AECOPD患者治疗前后血清bFGF水平均较对照组高,提示bFGF参与了COPD发病进程;②中度海拔低氧可刺激健康人血清bFGF表达,但对AECOPD患者的表达无影响,提示bFGF、低氧与慢性阻塞性肺疾病发病率、病情严重程度无明显关系;③血清bFGF水平对FEV1/FVC、FEV1%Pred无明显影响。
Objective To observe the effect of different altitude on basic fibroblast growth factor (bFGF) expressions of serum and the correlation between bFGF and FEV1/FVC% , FEV1% Pred in patients with acute exacerbation of chronic obstructive pulmonary diseases (AECOPD) before and after treatment as well as the healthy. Methods The serum contents of bFGF in patients with AECOPD before and after treatment as well as the healthy at two different altitudes were measured by enzyme-linked immuno sorbent assay (ELISA) method, then the data were collected and analysed. Results ①The levels of bFGF in patients with AECOPD before and after treatment were higher than those of the control group both in two different altitudes, the levels of bFGF in patients with AECOPD were decreased after treatment, and the decreasing of bFGF in patients with AECOPD after treatment at low altitude were higher than those of moderate altitude, the bFGF levels of the control group was lower than those of moderate altitude ( P 〈0. 05 ) ; ② Compared with the two groups respectively, there has no significant difference of the bFGF levels in patients with AECOPD before and after treatment; ③The FEV1/FVC%, FEV1% Pred in patients with AECOPD before and after treatment in moderate altitude group were significantly lower than control group, and the FEV1/FVC% , FEV1% Pred were improved after treatment, and there was no significant difference of the FEV1/FVC%, FEV1% Pred between the two groups at different altitudes; ④There was no significant correlation between expression of bFGF and FEV1/ FVC%, FEV1% Pred in patients with AECOPD. Conclusions ①The levels of bFGF in patients with AECOPD before and after treatment were higher than those of the control group at two different altitudes, it shows that the bFGF may take part in the process of COPD ; ②Hypoxia can stimulate the expression of bFGF of the healthy, but there has no effect on expressions of bFGF in patients with AECOPD, there has no significant correlation between expression
