摘要 目的 研究急性脊髓损伤后血清、脑脊液髓鞘碱性蛋白 (MBP)变化与脊髓损伤程度、预后情况之间的关系。方法 ELISA法检测 5 1例不同程度脊髓损伤患者伤后 2 4h内血清和脑脊液MBP含量 ,结合预后程度进行分析。结果 脊髓完全性损伤患者伤后 2 4h内即出现脑脊液MBP显著增高 ,较重的脊髓不完全性损伤 (ASIA分级B或C级 )伤后 2 4h内也出现脑脊液MBP增高 ,但各时间点增高幅度较完全性损伤组小 ;较轻的脊髓不完全性损伤 (ASIA分级D级 )和脊髓震荡不出现脑脊液MBP增高 ;各组血清检测结果与脑脊液结果一致变化。结论 受伤后 2 Objective To study the relationship between myelin basic protein (MBP) in serum or cerebrospinal fluid (CSF) of the disease in patients with acute spinal cord injury and severity and prognosis.Methods Serum and CSF level of MBP were measured by ELISA in 51 patients within 24 hours after acute spinal cord injury . The severity of spinal cord injury was retropspecxtively evaluated in terms of recovery 6 months after injury. Results Mean serum and CSF concentration of MBP raised significantly within 24 h after injury in patient with complete and incomplete spinal cord injury (ASIA B or C). MBP level in incomplete spinal cord injury was lower than that in complete spinal cord injury. However patients with spinal cord concussion or incomplete spinal cord injury (ASIA D), did not have increased MBP levell. The serum concentration was parallel to that of CSF. Conclusion Measuring myelin basic protein in serum and CSF in patients with spinal cord injury is helpful for assessment of severity and prognosis.
