摘要
目的探讨颅底骨折合并脑脊液漏的诊断与治疗。方法对68例颅底骨折合并脑脊液漏患者采用保守治疗,包括患者平卧或半卧位,吸氧、止血、预防感染,保持外鼻道、外耳道清洁通畅,密切监测生命体征,观察意识、瞳孔,行腰椎穿刺、腰大池引流;保守治疗无效采用手术治疗。结果 68例患者中,经一般保守治疗治愈55例,经腰椎穿刺、腰大池引流治愈10例,经手术治疗治愈2例,死亡1例,死亡原因为颅内感染。结论颅底骨折合并脑脊液漏的患者通过正确诊断,及时、积极的保守治疗,绝大多数可以治愈,预防和治疗并发症是降低病死率的关键。
Objective To discuss the diagnosis and treatment of skull base fracture combined with cerebrospinal fluid leakage.Methods 68 patients with basal fracture combined with cerebrospinal fluid leakage were provided with conser- vative treatment,including supine or semi-reclining position for patients,oxygen inhalation,hemostasis,anti-infection, clear and unobstructed passageway of external nasal meatus and external auditory canal,vital signs were closely moni- tored,consciousness and pupil were observed,lumbar puncture or lumbar cisterna drainage were used.Surgery was used if conservative treatment failed.Results Among all 66 patients,55 cases were cured by conservative treatment,10 cases were cured by lumbar puncture or lumbar cistern drainage,2 cases were cured by surgery,1 case of death,causes of death for intracranial infection.Conclusion Patients with skull base fracture combined with cerebrospinal fluid leakage, most of them can be cured by correct diagnosis and timely and positive conservative therapy.The key to reduce fatality rate is to prevent and treat complications.
