摘要
目的防范差错重复发生,保证临床用药的安全合理。方法对2012年全年静脉用药调配中心各个环节中出现的各类差错进行总结分析,并提出防范措施。结果通过防范措施的实行,静脉用药调配中心的差错发生率降低。结论通过制定严格的操作规程和配制规范,并加强防范措施,将差错降到最低,从而提高临床用药的安全、有效。
Objective To prevent errors occur repeatedly and ensure the safety and rationality in clinical medication. Methods Various errors that occurred in the pharmacy intravenous admixture service in 2012 were summarized and analyzed. Accordingly,the methods of preventing errors were put forward. Results The incidence of errors in the pharmacy intravenous admixture service was reduced by carrying out preventive measures. Conclusion These measures including the establishment of strict operating rules and the standards of preparation and strengthening the preventive measures contribute to minimize the error and improve the safety and effectiveness in clinical medication.
