气腹对大鼠肝内胆管上皮细胞凋亡和Bcl-2/Bax基因表达的影响

Effect of pneumoperitoneum on apoptosis and Bcl-2/Bax expression in intrahepatic bile duct epithelial cells in rats

摘要 目的探讨气腹所致缺血-再灌注损伤对大鼠肝内胆管上皮细胞凋亡和Bcl-2/Bax基因表达的影响。方法 SD大鼠40只,随机均分为五组,其中S组为假手术组,经脐插入气腹针但不充气;P1和P2组气腹压力为10mm Hg,P3和P4组气腹压力为15mm Hg,P1和P3组气腹时间为2h,P2和P4组气腹时间为4h。各组大鼠达到预定气腹时间排气后6h取血清测定肝功能;取左肝内叶肝组织,TUNEL染色测定肝内胆管上皮细胞的凋亡指数(AI),免疫组化检测肝内胆管上皮细胞内Bcl-2和Bax基因表达评分(IHS评分)。结果 S组大鼠血清丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)水平明显高于其它四组(P<0.01)。P2、P4组血清总胆红素(TBA)明显高于其它三组(P<0.05),而血清碱性磷酸酶(ALP)明显低于其它三组(P<0.05)。P2、P4组肝内胆管上皮细胞AI明显高于其它三组(P<0.01)。P2、P4组肝内胆管上皮Bax基因表达IHS评分明显高于其它三组(P<0.05)。结论长时间气腹可引发肝内胆管上皮细胞内的Bax基因高表达,使得Bcl-2/Bax表达水平发生变化,导致肝内胆管上皮细胞凋亡增加。 Objective To investigate the effects of carbon dioxide pneumoperitoneum on apoptosis and Bcl-2/Bax expression in intrahepatic bile duct epithelial cells in the rats. Methods Forty healthy male SD rats were randomly divided into five groups (n=8 for each group) and subjected to the different insufflation pressures or pneumoperitoneum time duration: no gas control (group S), 10 mm Hg-2 hours (group P1), 10 mm Hg-4 hours (group P2), 15 mm Hg-2 hours (group P3) and 15 mm Hg-4 hours (group P4). Liver function was evaluated using an automatic biochemical analyzer. Apoptotic indexes of intrahepatic bile duct epithelial cells were measured by TUNEL in 40 biliary specimens, and the expression of Bcl-2/Bax proteins in intrahepatic bile duct epithelial cells were determined by immunohistochemistry, respectively. Results AST and ALT in groups P1,P2, P3 and P4 were significantly disturbed compared to those in group S (P〈 0.01). ALP and TBA in groups P2 and P4 were significantly higher than those in other groups (P〈 0.05). Apoptotic index of intrahepatic bile duct epithelial cells groups P2 and P4 were significantly higher than the other three groups (P〈0. 01). There were no differences in the expression of Bcl-2 proteins in intrahepatic bile duct epithelial cells among the five groups, but the expression of Bax proteins was significantly increased in groups P2 and P4 compared to the other three groups (P〈 0.05). Conclusion Longer time duration pneumoperitoneum increases apoptotic indexes of intrahepatic bile duct epithelial cells, which may be attributed to the higher expression of Bax proteins.
