Investigation and analysis of nosocomial infections in gastrointestinal surgery department

摘要 目的分析胃肠外科医院感染的发病特点及相关危险因素,探讨有效的医院感染的预防控制措施,以降低医院感染率。方法对胃肠外科2010年1月-2012年6月全部住院患者采取前瞻性和回顾性方法进行调查。结果 1673例住院患者中发生医院感染101例、128例次,感染率为6.04%、例次感染率为7.65%,其中手术部位和下呼吸道感染例次占全部感染病例的74.22%;病原菌分布中细菌占85.84%,真菌占14.16%,大多为皮肤和肠道正常菌种;坏疽性阑尾炎伴穿孔和消化道穿孔感染比例较高,分别达13.43%和11.22%,其次为肠梗阻和消化道恶性肿瘤,胃肠外科发生多重感染比例高达16.83%。结论胃肠外科感染以手术部位和下呼吸道感染为主,控制手术部位和下呼吸道感染,可有效降低胃肠外科医院感染率。 OBJECTIVE To analyze the characteristics and the related risk factors of nosocomial infections in gastro-intestinal surgery department, and to discuss the effective precautions and control meausres of nosocomial infections so as to reduce the rate of the infections. METHODS All patients between Jan. 2010 and Jun. 2012 in gastrointestinal surgery department were investigated using prospective and retrospective methods. RESULTS Totally 101 cases and 128 case-times of nosocomial infections occurred in 1673 cases of hospitalized patients, and the infection rate was 6. 04%, the case-time infection rate was 7. 65%. Amoay nosocomial infection cases, surgical sites and the lower respiratory tract infection accounted for 74.22 %. Main pathogenic bacteria were normal strains of skin and intestinal tract: bacterial 85.84% and eumycete 14. 16%. Nosocomial infection rate of perforating appendicitis and alimentary tract perforation were high, 13.43% and 11.22%, respectively. And the nosocomial infection often occurred in patients with intestinal obstruction and alimentary tract malignancy. The rate of multi-ple infection was as high as 16.83%. CONCLUSION Surgical site infection and lower respiratory tract infection were the main infection sites in gastrointestinal surgery department. The nosocomial infection rate can be decreased by controlling surgical site infection and lower respiratory tract infection.
