腰椎骨关节炎影响磁共振信号改变的临床研究

摘要 目的通过观察腰椎骨关节炎腰痛急性发作时的磁共振信号改变,评价小关节或其周围组织高信号区对腰椎骨关节炎急性腰痛的诊断价值。方法收集腰椎骨关节炎急性腰痛患者50例和正常体检者20例的磁共振影像学资料,观察其腰椎小关节及周围组织的信号情况并进行综合分析。结果 50例腰痛患者中有42例出现腰椎小关节或周围组织高信号反应,20例正常体检者中出现腰椎小关节或周围组织高信号8例,经统计学分析,在腰椎骨关节炎腰痛急性发作时,小关节及周围组织的高信号对于临床诊断有意义(P<0.05),高信号区所处腰椎小关节位置则无(P>0.05)。结论腰椎骨关节炎患者磁共振上小关节或周围组织局部高信号与腰痛急性发作有关,对腰椎骨关节炎急性腰痛的诊断具有辅助意义。 Objective To evaluate the diagnostic value of the high signal of facet joint or peripheral tissue in acute lumbar pain of lumbar spinal arthritis by obsmwing the changes in MRI during the acute attack of lumbar pain. Methods MRI data of 50 cases of acute lumbar pain and 20 cases of healthy persons were collected. The signal of facet joint and peripheral tissue was obsmwed and analyzed comprehensively, Results 42 of 50 cases of acute lumbar pain and 8 of 20 normal cases had high signal reactions in lumbar facet joints or peripheral tissues. By statistical analysis, during the acute attack of lumbar pain, the high signal of facet joints and peripheral tissues was significant in clinical diagnosis ( P 〈 0.05 ). The location of lumbar facet joint at high signal zone did not present the significance on clinical diagnosis( P 〉 0.05 ). Conclusion The- high signal of MRI in local facet joint or peripheral tissue is relevant with the attack of acute lmnbar pain in the patients of lumbar spinal arthritis and provides the assistant significance in the diagnosis of acute lumbar pain of lumbar spinal arthritis.
