摘要
目的 :了解烧伤感染的细菌学特点 ,为临床合理用药提供实验依据。方法 :按全国临床检验操作规程进行 ,药敏试验采用K -B法。结果 :共检出 2 30株细菌 ,其中G- 杆菌 79 8% ,G+ 葡萄球菌 2 0 2 %。结论 :烧伤感染中以铜绿假单胞菌、金黄色葡萄球菌、阴沟肠杆菌、醋酸钙不动杆菌、粪肠球菌、大肠埃希菌、变形杆菌及肺炎克雷伯菌 8种细菌为主。对G- 杆菌引起的烧伤严重全身性感染第三代头孢菌素及亚胺培南应作为首选药物。而第一、第二代头孢菌素、青霉素类抗生素及复合制剂药物应在明确病原菌及药物敏感试验结果后指导应用。万古霉素应作为治疗烧伤严重全身性耐氧西林金黄色葡萄球菌 (MRSA)感染的首选药物。
OBJECTIVE:To investigate bacterial type in infection of burn and provide evidences for clinical treatment. METHOD:bacteria were detected by national lab standard. Drug sensitivity test was conducted by method of K-B. RESULTS:230 strains of bacteria were detected, which 79.8% were gram-negative bacillus and 20.2% gram-positive staphylococcus. CONCLUSION:8 bacteria as verdigris pseudomonas, staphylococcus aureus, enterobacteriaceae, calcium acetate, fecal enterococcus, escherichia coli,metatypic bacillus, klebsiella pneumoniae, et al, are the major infected bacteria. Systemic infection caused by gram-negative bacteria must take the third generation cephalosprins and imipenem-cilastatin sodium as the first choice. The first and second generation cephalosprins, penicillin and other compounds are recommended with specific bacterial infection and sensitivity test. Vancomycin is recommended for systemic infection caused by oxytolerant staphylococcus aureus.
