摘要
目的 对比腹腔镜手术、后穹隆切开引流术治疗盆腔脓肿的临床价值.方法 回顾分析2008年1月～2013年6月本院妇产科收治的46例盆腔脓肿患者,24例采用腹腔镜手术治疗,22例采用后穹隆切开引流术治疗,对比两组手术时间、术中出血量、术后体温恢复时间、肛门排气时间、留置导尿时间、平均住院天数.结果 后穹隆切开引流组患者症状、体征明显好转,但2例因脓肿位置较高,脓肿数量多,改行腹腔镜手术;腹腔镜组24例均顺利完成手术,切口均一期愈合;两组手术时间、术中出血量、肛门排气时间、留置导尿时间、术后住院天数、并发症发生率差异有统计学意义（P＜0.05）,两组术后体温恢复时间差异无统计学意义（P＞0.05）.结论 后穹隆切开引流治疗盆腔脓肿经济实用,操作简单,能避免患者手术痛苦,但适用于单纯感染性低位脓肿或富颈旁脓肿;腹腔镜手术创伤小、恢复快,是治疗盆腔脓肿的理想微创手术方法,但费用较高,要求术者有丰富的腹腔镜操作技术.
Objective To compare the clinical value of laparoscopic surgery and posterior fornix operative drainage in treatment of pelvic abscess.Methods Clinical data of 46 patients with pelvic abscess treated in our hospital from January 2008 to Jun 2013 were retrospectively analyzed.24 cases of patients were given laparoscopic surgery treatment,22 cases were given posterior fornix operative drainage treatment.The operation time,the volume of hemorrhage temperature recovery time after operation,anal exhaust time,indwelling catheter time and the hospital tion in two groups were compared.Results Symptoms and signs recovered obviously in posterior during operation, stay after operafornix operative drainage group except that 2 cases with higher location and larger number of abscesses changed to laparoscopic surgery.The treatment of laparoscopic surgery group was completely successful,and the incisions all healed in first intention.The operation time,the volume of hemorrhage during operation,anal exhaust time,indwelling catheter time,the hospital stay after operation and complication rate between the two groups were significantly different （P〈0.05）.But temperature recovery time after operation showed no significant difference （P〉0.05）.Conclusion Posterior fornix operative drainage is economical,practical and simple,it can make the patient free from the suffer of the operation.But it only applies to the purely infectious low position abscess or pelvic abscess near cervix uteri.Laparoscopic surgery has the virtues of small trauma and rapid recovery,it is a ideal minimally invasive operation method for the treatment of pelvic abscess.But it costs more,and requires rich laparoscopic operation technology.
